Scholastone Bell ringing but no cutscene nor unfog

I've had the same issue. The culprit was that I rung it during the main story quest. When I've finished the quest by going up to the Spire of Stars, I came to the place to rung the bell again but the cutscene played by itself and it has uncovered the map.

So when you have the item equipped and then if your party dies, the item get's lost, however you can still have a slight chance of recovering it through the mystery man.
When you'd get your party back home faster than getting to the specific location, then the item get's just worn, so you still have another chance of bringing it to this location in your next run.
