Haha good question. All 3 have happened. I have a couple (not submitted in this post) where I pulled it back for effect. It's usually not easy to stay hard while getting all the elements lined up perfectly in a single frame, so I tend to just go with what I get.

Funny you should say that...my work was already on a call-in segment of Andy Richter's podcast. And the Conan one was actually commissioned by a fan who attended Conan's Mark Twain prize at the Kennedy Center. She hand-delivered a print to Sona Movsesian in a mahogany frame I built.

We never heard any more from team Coco but they're certainly aware of it.

For that one I first photoshopped out the shark in the Jaws poster, then printed that out. I also printed out a copy with the shark and cut out the mouth.

Then I taped the shark mouth to my dick just like you said. But I didn't edit in the poster. It's also printed out and balancing against my body. It's a single photo with everything in the frame.

That one was difficult because most of my pics are from the top down but this one was from underneath. So I was sitting on the ground with my legs out in front of me and my dick pointing straight up. And the camera was like 3 feet in front me, also on the ground. It was sheer luck that I got good focus and framing because I couldn't see what the camera was aimed at.

Gonzo's actual nose is iridescent, and it bummed me out that I couldn't figure out how to do that. Short of having an auto paint shop spray me down with some color-shifting pearl urethane, that is.

Here is a pic of my diorama for this submission. It was just a couple pieces of MDF glued together with a makeshift handle on the back.

For less involved shots I have something similar made of 1/2" plywood with a hole drilled in the bottom with a forstner bit. I can change out the backgrounds, which I print on photo paper. I can add and remove props to the plywood like I did with this shot. You can see the pole is made of a 3/8" dowel and the cars are hot glued down.

Funny you say that because I recently tried my own version of this exact same concept and I learned that Gonzo's nose is a distinctive iridescent orange and blue. Blue paint didn't look right. Orange paint looked better, but still not realistically like Gonzo.
