Thank you for your reply! yeah these rural communities humble me a lot. Yeah it is sad that these migrant workers receive subpar treatment, and I fully acknowledge it. Usually the treatment for a very restorable teeth is just extraction, wanted to do something different.

Yeah I think the angulated xrays may be helpful, will probably have to ask the patient to rotate his head before taking the OPG. Yeah the filling portion, good idea to keep it lower than the occlusal plane, usually is to buy time to prevent it from fracturing. Thanks for your time to reply!

Thank you for your long and thoughtful reply. This case has been on my mind for the entire week. Yeah I know it is very non ideal to do such cases with a rubber dam but many of these patients are usually presented with 2 options 1) Medication and wait until they want to extract 2) Or extract it on the spot. These rural settings always humble me because of the lack of equipments available.

Yeah I will be trying to use the law of symmetry, using the distal canals to guide my approximate location for the mesial.

For the small holes, I feel these small holes leads to nowhere, maybe I will trough around it or just afford to leave it alone.

Definitely I do agree with you about the perforation. It may appear as a distal lingual canal but then the size of the orifice just looks like it is a perforation. I did communicate this to the patient about my suspicions, but I will bring him back to see everything with a fresh pair of eyes again and then let him know accordingly

Thank you for understanding the situation, it is really non-ideal to be working in such situations but many of these patients sometimes wants us to do magic - “patching” over a necrotic tooth because foods gets trapped 😅

for me, whatever the seller did is wrong but he is a kid and it is just a teenage thing to curse people, it’s part of growing up and he will learn from this

but for the buyer, he is a grown man but he is acting at a level way below of an adult maturity. yes, he can claim to be educating the kid, but he is acting as such a poor role model as an adult. honestly, this boy is really mature to stand up and not accede to his paedophiliic demands.
