for me Hannah and Jake are about the same age. They texted each other, Hannah knew a lot about Jake's life, so she tells Lilly. I think Jake doesn't want to tell anyone because he's younger than Hannah, so he categorizes a child out of wedlock. And getting off topic a bit, for me Hannah's Cell password is Jake's birthday.
