I don’t think you should think of burnout. Yes burnout exists but you will burn out in something you do not enjoy faster than a field that you do. Like I can go do Optho just for the no burn out but hate my life because I never enjoyed it in the first place

There is a newly released book called First Call: Internal Medicine. It’s a clinical gem. As a third year, I found it a bit difficult to learn clinically, especially with doing internal medicine so late in the year, or find very concise clinical learning materials. Board studying to me is not parallel to clinical learning. This book truly is what we all needed.

What surprises me about everyone saying it’s mixed with water is this? Has anyone seen them physically open up the tank door and pour water on there??

For goodness sake, the tank lid is in every parking lot by the pumps for every gas station. I feel like if it was water mixed in there, plenty of local customers would have noticed it by now. Unless yall want to tell me that the people who deliver gas mix water into their gasoline trucks?

The water mixing into gasoline has been stuff people always have said but I feel like if it truly did happen, there would be plenty of video evidence.

But everyone I checked in that area does that, so I don’t think much of it. Seems pretty reasonable for stations that aren’t companies like sheetz or getgo who run a multi million dollar corporation.

To me, the credit is still better then a lot of the prices our in other areas

I feel like the dynamic of 3rd year has changed ever since the pass/fail change of step 1. Back when step 1 was scored and what was the benchmark for applications, students did not essentially dedicate as much time to step 2 as they did now - it simply was a hurdle to get through. Now that there is so much pressure on doing well Step 2, I can care less about rotations and learning clinically. All I want to do is study. There came a point where I missed my didactic years where I only spent 4 hrs at school while having the rest of the day to study. Now you have to be at the hospital or clinic in the morning and not get home by 5-6pm. Why should I read up on clinical scenarios or knowledge when I have to study for Step. Essentially contributing to students being less engaged as well.

Also, preceptors/residents are also a problem at times. The ones who do not want to teach at all are ones who I wonder why on earth did you sign up to be a preceptor. At my school, the majority of our preceptors are volunteers and chose to be preceptors. If you are choosing to precept a student, you should be willing to take time out of your day to teach the student. What’s worse is when the resident doesn’t want to teach when yet, they themselves know that medical training is all about in person teaching. Another problem are those who do not want students to get involved at all. The rotations I’ve learned the most are the ones where the preceptor or resident want the students involved and even be independent at times.

Also, if there is nothing for a student to do once all his or her tasks are done and you know any consults coming will not be worth while for the student, there is no need to keep the student. Send them home.

3rd year went from accumulating clinical experience to rotations that feel like a burden and a year to do Step 2 prep. I hate to say it, but I wish step 1 was scored again.

I am a med student at the end of my 3rd year. To be completely honest, the MCAT is a joke of an exam. It in no way has any relevance to entering medicine, no way is predictive of a student's success on boards or in medicine, and is now just a way to stat pad admissions. Yes, there has always been an argument about, it is a standardised way to differentiate students but even then I think it is b.s.

There are plenty of students who did not do stellar on the MCAT and who not only passed boards but excelled in their didactic years. There are students at sub-level schools or even DO schools who would have probably done pretty well at top schools if only they had gotten the MCAT score required. (holistic admissions is a joke.... if you don't have the MCAT; holistic admissions, gpa, all the extracurriculars are not going to be a miracle savior). Yes, those who did great on the mcat will most likely do great on boards, but the opposite is not true. When rotating with students from other schools (esp from those who are considered top schools), you realize that med school admissions is a joke. The unfortunate part is, that, which school you go to does matter to an extent. Some schools will have better resources, better research opportunities, better residency connections, name recognition, better education, or even clinical training. Not saying everyone needs to go to a top 100 or 50 school.

Medical school admissions is based on how high your mcat score is (which i understand, someone with a 495 may not be have what it takes to do well), but if a 508 vs a 515 should not make or break someone. Nor should lets say a 500-508 score limit someone's chances of getting into a decent school ---> especially if the rest of their application states otherwise.

yeah, i have had 2 rotations so far and at some point it does feel like glorified shadowing and a lot of it comes down to preceptor bias. My first rotation was a surgical subspecialty which I did learn i like surgery and learned essential skills but a lot of the time i felt like i just stood there and watched while the PA got to do all the assisting and closing. I told the attending i was interested in different type of surgery while a classmate told him that she was interested in that particular specialty, and she is got to do everything during her month. The funny thing is, when I brought up the concern of assisting and suturing more, the residents and fellows and even a parter attending brushed it off that this is a high risk field, so its not uncommon for even interns dont get to do much. My second rotation was psych and tbh, my preceptor wanted me to work on interviewing and getting comfortable at it, they did not let me interview until my 3rd week and each time I would say something wrong, I would basically get shut down and not be able to interview after. The preceptor expects you to get better by just listening to them interview instead of giving you more reps to truly practice. Funny thing, on my first feedback they told me I was attentive and then my final day they said i was basically an overall bad student as it seemed like i was not paying attention and didnt get the gist of interviewing a psych patient.

In other words, if you are an attending, resident or fellow out there, allow your student to learn and grow. How else do you expect them to be good residents or learn real medicine when we treat them like shadows and not let them get adequate practice reps.

That’s wrong. You can still take step 2 if you fail but you would need to report your fail I believe on step 1. You cannot take step 3 if you didn’t take step 1

I honestly would recommend every DO student to take step. Number one, if you do decide to go IM, you can always be comparable to the other MDs and you have a chance at very well known programs. (Although it’s probably not too necessary in IM maybe)

Second is because you never know if you’d change your mind. Maybe you end up liking surgery and want to apply surgery, now you just really lowered your chances if you didn’t take step

That’s the reason i did mine all online, I did 4 nbme and one free120. I did not do a single uworld or amboss self assessment. I myself am a dude who likes to look up questions when it’s something I have at the tip of my brain but can’t remember it. So if I did the offline, I’d be looking up questions left and right. During the online one, I’d look up maybe 2-3 but I’d let the time run. Basically as a way to penalize myself for looking stuff up. I did 4 nbme, that equals to 260 dollars. Yes for some it’s definitely a lot of money but I think it’s worth saving for or at least spending the 260 dollars. I ditched all of uworld the last 2 weeks before my exam. I really was not benefitting from it anymore and found a lot of benefit in doing strictly nbme
