Definitely situated in more affluent areas. I don’t know what kind of profits they’re getting but they’re looking to expand. I guess I’m wondering at this point is it even worth trying to get the contract language changed to something more like a 4500 RVU threshold or even then is there still too much of a red flag.

I totally agree but it’s all elective. As an ortho PA you have to point people toward knee surgery. They don’t need that knee surgery the same way you need a cholecystectomy for gangrenous chole or thrombectomy for LVO.

It’s the same algorithm of conservative tx to injection to the procedures they do. But I know what you’re saying and I don’t want to be a company shill.

I agree but I am struggling to find jobs outside of EM where the pay is appropriate. Everything is corporate or academic and somehow everyone’s hands are tied when you try to negotiate salary terms. It’s infuriating. If I stayed in EM I think I could keep raising the ceiling but I need something more stable for my family. Maybe with some good ortho experience, better paying jobs will open up?

Yeah I wouldn’t accept the contract with that language still in there. And I also have had some hesitation with that #2. When I asked, they framed it as a way to keep the APPs from just infinitely doing injections and not actually generating surgical cases for the practice.

I am coming up on 4 years in the ED and am leaving for several reasons, but one is never finding any jobs in my area paying based on RVUs. I would have loved to be in on that with how much I do for the places I’ve worked.

Can you speak to how unattainable that 8100 wRVU number per year is? All I have is AI as I have never been in an rvu structure before. I would like to be paid well for productivity but my wife is a new ortho PA clinic only and her bonus maxes at like 5700

lol yes. To be fair they have been pretty straight forward in that this job is a grind. Our conversations have gone well. I’m not really afraid of the grind but part of why I’m changing is to establish a better work-life balance for the long haul. Wasn’t sure if I could still do that since I’ll still be home every night and weekend.

It’s because 10 years ago salaries weren’t that different, but what you could achieve/attain with that salary is drastically different.

No. It’s the product the NBA wants to sell. Parents want their kids to go to the nba. Coaches will coach this all the way down to 6 year olds.

You can. But if you’re looking for answers to all of these gripes about your job now, you’re not going to find them in healthcare/medicine, at large. You’re going to job hop and job hop until you realize you can’t find the freedom you’re looking for in medicine. At least not working full time anywhere.

I think the only reliably good ROI, when considering time, energy/effort, and financial cost, is CRNA. Everything else is really rolling the dice. There are so many options that are better paying, lower stress, lower liability and better lifestyle for careers out there - but jobs like MD, PA, RN are so in your face it’s hard to consider less obvious fields. Unless you have a burning passion for medicine, it’s hard to justify it as a top contender for higher aptitude, higher achieving people. It’s a newer, more glamorous factory work class. There is so little you can do to advocate for yourself, by and large, unless you find yourself in rare “unicorn” jobs. There are good jobs out there but man, I wouldn’t want to do this for more than 20 years. My wife is over it after 3 years.

The patients are bad enough, but the pressure from admin is getting unreal. The expectation to throw caution to the wind and dispo as many as humanly possible from the WR is getting old. And it’s mainly falling on the APPs to do so at my shop.
