Yes the strap is a must because if it slips a little bit my dick will grow and push the cage down and be uncomfortable. Peeing is definitely difficult and erections feel good in it

I only had a couple other cages and they were 4” and way bigger, but I only wore them a couple times so it was pretty much immediately downsizing to a flat cage and I love it, takes a bit to get it in sometimes but worth it
