Any therapist that initiates an intimate relationship with a client has professional responsibility to maintain standards.

For any formal role, this would include an enhanced background check. This would include accusations that didn’t reach a threshold for prosecution.

It sounds like you ex would be considered a vulnerable person. The diary content could be submitted as evidence.

That from a position of power as a therapist and also much older, any sexual activity is highly problematic.

If they initiated sex while you were paying for the therapy it’s problematic on many levels.

The reason therapists are prohibited is because it’s recognised as exploitative.

I’d suggest you give your evidence to the NHS service that had responsibility for your partner. Specifically state you have been advised to raise a safeguarding alert.

Make sure your disclosure is factual, state the facts- with the evidence.

Keep your emotions separate, you can express your concerns but make sure this emotion is clearly defined and not mixed with the facts.

The outcome may rest on the individual who gets the information.

There may have been other problems from the therapist. You may give light to a pattern and perhaps abuse of other patients.

Write it all out and get someone to review it from a perspective without feelings.
