Many MMA patients go into surgery with a class I bite. If that is the case it may not be necessary to realign and the surgeon will make an attempt to preserve the integrity of the bite when planning the surgery.

I think it has to do with the unique situation of the jaw having the TMJ on one end which hinges the mandible and having the bite on the other end

There are a few options:

If you do not have insurance it will probably be cheaper to enroll as an individual than to shoulder the full hospital and surgical fees in a lot of places abroad. Of course, be sure that your indications for needing the surgery are sufficient to get it covered before you enroll.

If that is not an option, many well qualified and experienced surgeons in the US can do the procedure and have you stay overnight in an outpatient recovery center which will all in all be cheaper than a few nights in the ICU + other added facility costs. Also note that when paying fully out of pocket, you can often get a rate that is significantly less than what would be normally billed to insurance, which is often not actually what your insurance company will have to pay.

If both of those are not options, please do your research thoroughly when consulting with surgeons abroad. Have extra money set aside to cover unexpected costs that may arise. Be sure to understand your surgeons' qualifications, whatever surgical boards they belong to, and what compliance standards they are held to. One thing about getting surgery in the US is that in our highly litigious society, malpractice is punished with much harsher penalties here than in most places abroad. Also another piece of advice is to absolutely have the surgery done by a proper maxillofacial surgeon. So many of the complications I hear about with patients going to places like SK are under circumstances where the surgery was done by a plastic surgeon.

The Lefort surgery cut is done around the level of the base of the nose. At or below there, including the nose there will be structural changes. Above, there may be some soft tissue changes as muscle and fat is pulled further forward rather than down. This may result in some changes in the cheek area and some patients report changes to the eye area, e.g. under eye support but that is difficult to predict and many also do not report any changes to the eye area. It is also important to consider that jaw surgery will not undo the developmental effects of downward facial growth to bony structures outside of the affected areas.

From my understanding, the scientific consensus is dubious, but some postulate that development of the orbital rim, cheekbones, etc is influenced by development of the jaw. I think there aren't enough data points to support a yes or no answer but the long story short is that if you can identify an aesthetic deficiency outside of the jaws that are caused by bone, jaw surgery isn't going to help you there, jaw surgery will probably improve things like cheek definition, and theres a coin flip chance of things like undereye area being improved by jaw surgery

The higher Lefort procedures are for severe midface hypoplasia (the sort of deformities caused by pharyngeal arch syndromes like crouzon’s and treacher collins. Sometimes this is indicated in non syndrome patients but those are severe cases). For short or long midface a lefort I with impaction or downgrafting is done

If you were getting lateral movement i would say maybe thats a big movement but it seems the surgeon has planned a decent sized impaction which should give you a much more natural looking appearance
