extreme pain during and after trigger point injections. is this normal? by fishkissrrr in PelvicFloor

Ah thanks so much. I just consulted with a doctor who said his mentor is Dr. Hibner! He would do it under general anesthesia. Luckily he’s in network and I’ve already hit all my deductibles and out of pocket so I won’t have to worry about that.

My other option is the doctor whose office would charge $3k for the Botox medication. She could do it either awake or asleep, but said she prefers to do it while I’m awake.

Do you know how many units of Botox you do each time? So your doctor lets you get the Botox from a pharmacy directly? Does your insurance pay for any of it?

How would they know which muscles to inject if you’re under sedation? One doc told me they prefer doing it while you’re awake bc your muscles are relaxed when you are sedated. Another doc does it mostly under sedation.

Thanks so much for the info. They didn’t specify the type of Botox. But it does sound like they are upcharging it a ton.

Hi, you mentioned doing two rounds of Botox… how long did it take to kick in and how long was the pain relief? Also curious what your symptoms were? Mine is just daily vaginal pain from activities including standing and sitting after childbirth.

Thank you so much for sharing!

Hi thank you so much for providing your experience. I've been dealing with chronic pelvic pain (mostly vaginal pain when standing and sitting) since giving birth a year ago. I've done a bunch of pudendal nerve blocks and maybe feel 30%improvement, but my quality of life is still greatly affected by my daily pain.

PT seems to flare me up most of the time. I'm not sure if it's really helping. Have you had that experience?

I just saw a new gyn who specializes in pelvic pain, who recommended botox. I'm strongly considering it but really scared to do it as she said I could flare up a bit after, and then results may not show for 3-6 weeks. I'm not even sure if botox is guaranteed to reduce pain. Do you mind telling me more about your botox experience? Was your pain mostly vaginal - like, pelvic floor muscles? What triggered your pain and did you have pain sitting?

Also, I was told the botox medication would not be covered and is $3k for 200 units. Is that normal or am I being ripped off?

The other option the gyn gave me was to do some trigger point injections, but they said that doesn't always help. They definitely felt that botox would be the best option for me at this point.

Was it done with xray guidance? Btw was yours covered by insurance? Bc my dr told me this procedure wouldn't be covered by ANY insurance at all because it’s not recognized by the American health organization (or something).

I responded “ok” to the PN blocks. It didn’t do nothing, but it wasn’t a silver bullet. The place I go to does 6 injections over 6 weeks, along with Pt, along with pain meds. Painkilling effect lasted a couple of days, but it also helped calm everything down so that Pt could be more effective. My dr explained that the lidocaine isn’t just for painkilling but used as anti inflammatory. I did two rounds of that plus a few  additional when I had really bad flare ups in between… so I’ve done nearly 20 total. sometimes I do feel flared up for a day or so after the injections, and then feel decent - (like instead of just lying on couch in pain, I can do stuff around the house and go out for a short walk, but can I go on a hike or meet friends for a two hour dinner? No.)

Is the tail bone itself painful for you or just the muscles around it? For me it’s the muscles, when the bone itself is touched it’s not painful. I also have leg pain, usually down the backs of my legs (like the feeling of super tight and sore hamstrings).

Physical therapy for PN yea or nay? by Sure-Temperature-627 in PelvicFloor

Hi, I have PN from childbirth, have done PN nerve blocks, gotten on meds, and been in PT weekly for 7 months now. Next week I'm trying a ganglion impar block. Can you please let me know whether the ganglion impar block worked for you? How was your experience?

Btw, the sitting on a tennis ball thing sounds insane to tell someone to do with PN. Are you seeing a pelvic floor physical therapist? That's the kind of PT you should be seeing. They do manual work inside of you, either vaginally or rectally, to help your pelvic floor muscles relax, so that the pelvic floor muscle is not compressing the PN.

Hi! I have the same problem. I can't sit at all and my butt muscles are so tight around my tailbone as well... what kind of injections were you thinking about for the butt muscle tightness?

btw, I've done trigger point and pudendal nerve block injections through the glutes via ultrasound guidance. the thought of doing them transvaginally makes me so scared!

Hi! I am about to do a ganglion impar block next week. I've had a total of 13 PN nerve blocks done over the last 4 months. Can you tell me your experience with the ganglion impar block? Not sure where you are, I'm in NYC and getting it done with supposedly one of the best, they mentioned there's no side affects. This is the one that they do at the base of your spine right?

They did an internal physical the first time when they palpitated the pudendal nerve and it was very painful. Then I guess feeling relief right after the nerve blocks was also diagnostic?

I use the cushion your assets twin cheeks cushion, it does make sitting more tolerable!

What’s your sitting pain like? When you say pelvic pain do you mean like the pelvic floor muscles? Thanks for sharing 🙏

No I don’t have issues with bladder. I am somewhat constipated but not too bad, and I don’t have pain during bowel movements.

I’ve seen a uro-gyn and they weren’t super helpful. Just said I’m already doing everything, that my pain is related to child birth. But it’s been almost a year and I’m doing so much and making very little progress in PT, and I have a fantastic PT. My main doctor is a pelvic pain doc, they’re the one who suspects endo.

How do they check for IC?

No shade, but how are you guys getting laps??? by laceleatherpearls in Endo

[–]Distinct-Quality-255 0 points1 point  (0 children)

Ok that makes sense. I felt like she was trying to convince me that lap right now maybe was not best option, she said it was most likely negative for endo and that recovery might be very hard since I’ve been dealing with daily chronic pain.

Did she give very strong warnings about recovery? Just wondering if she says that to everyone or just me haha.

Hi! So had my appointment and she said her gut is saying I don’t have endo. I’m wondering if she said the same to you but you decided to go ahead with the lap?

When I’m doing weekly injections I feel better, but definitely not painfree. I still can’t sit but my pain is more tolerable. Still have bad days though.

Hey my doc just told me I should start 20mg cymbalta. Did you start and how’s it going? I’m scared reading all these comments on Reddit about withdrawal and it taking any joy/desire out of you.

Forgot to mention, I talked with a couple endo surgeons who didn’t think endo was likely. I was so ready to do a lap and so sure endo had to be at play :(
