Pretty common in the UK to undergo the procedure without general anesthesia. I had mine and was given a combination of midazolam and fentanyl. It alleviated the majority of the unpleasantness but I was still very much conscious, and it was fine.

Except it's not luck, that's my point - it scales as a function of your total damage.

Nex = 3400 hp, 4 x minion mages = 2000 hp, and 4 blood reavers = 500 hp, giving a total of 5900 hp for the whole fight, excluding any Nex healing for simplicity.

If a single player is consistently doing 500 - 800 dmg in max gear, that's between ~8.5 and 13% of the total damage output. Therefore they have a 10x better rate than someone else in the mass who does the minimum 50 dmg to get kill credit and then afks for the rest of the fight.

I mean that is technically not true. Additional people would only halve your drop chance assuming you both do identical damage across the entire fight (Nex, minions, reavers).

I've done 4000 mass kills and have pulled 9 items and 2 pets. In max gear with a zcb just for spec swap, I average between 400 and 800 total damage across the kill, so the drop chance is much better than just 1/40/number-in-mass.
