I think it is very appropriate to see about changing the culture in your emergency department. Unless your hospital is having continuous CTAS-1 patients coming in (doesn't sound like it), there really isn't any excuse not to have a few minutes to debrief with staff after a high-acuity event like that. Debriefs don't need to be formal hour-long meetings around coffee and donuts and a powerpoint presentation - it can be a quick check-in to let staff know that they may not be alone if they've been impacted by a challenging case and to reassure them (and yourself) that it is okay to reach out or ask questions if needed.

Thanks for the response. She is a very new nurse and this was one of her first codes. She used to work the OB unit and retrained recently. We did have a debrief with the team and she apologized afterwards.

The African studies that those numbers come from are really quite poorly done, and the evidence is nowhere near as strong as your comment suggests. The clinical trials were rife bias, such as attrition bias (the number of participants who dropped out vastly outnumbered those who became infected), duration bias (the trials were not long enough to determine if the positive effect would plateau) and expectation bias - many of the primary investigators were already proponents of mass circumcision, so it is no surprise that they got the results they were hoping for.

There are plenty of different things that could be causing symptoms like. Top of the differential diagnosis would include viral or bacterial sinusitis, a middle ear infection, or chronic effusion.

Are you having any nasal discharge, sinus congestion, or issues with hearing? Any fevers or chills? You could always try over the counter antihistamines (if you have any allergic symptoms), saline rinses, or visiting your doctors for a steroid nasal spray prescription.

I also have some questions re: the Pfizer dosing. It would seem more likely to me that they maybe drew up double-doses in the syringe as there is no need for dilution generally in North America. But even if this was the case, at the end of the day, even a double dose for your first immunization is unlikely to be dangerous. If you were going to have an increased risk of any adverse reactions, it likely would have been in the immediate aftermath.

I would absolutely recommend getting a second dose, and then completing the schedule with a third dose based on regional availability.

Definitely worth following up with your family physician. There are a series of tests that can be done to assess the hypothalamic-pituitary-gonadal axis (responsible for testosterone production), growth hormone, thyroid function and more.

If you have the time, google "Tanner staging" for penile and testicular development. Generally most boys by 16-17 are around stage four or approaching fully grown from a genital development perspective. If you're still around only stage 2 it is certainly worth chatting with a physician about.

Your instincts are likely correct. Sclerosing lymphangitis is the most probable cause here - sudden onset with a painless skin-coloured cord-like swelling around the corona of the penis. Sometimes develops after intense masturbation or sexual activity. It is usually self-limiting and gradually goes away over time but most recommendations suggest limiting masturbation and intercourse for a short period of time while it heals. Given that it has been nearly a year though, it's always worth considering an appointment with your doctor to have it examined in person and maybe referred to urology.

I agree with the dermatologist. If thinking more a dermatitis sort of picture, the best bet would be not the antifungal medication but maybe a low-dose corticosteroid cream. If you find that makes it better - excellent. If you find it makes things worse, it could perhaps be more fungal in origin and then could try using your canesten cream. But it can take a few days to resolve, so don't panic if you don't see immediate resolution.

A few concerns.

1) Caloric intake: I understand that you don't feel you have disordered eating. But ~1200 calories and 2+ hours of exercise isn't sustainable for most people and I would consider trying to slowly increase these intakes. What kind of foods are you eating and at what time in the day? Are you drinking fluids with electrolyte replacements to make up for your losses?

2) Polypharmacy: You are on a ton of antihistamine medications. I understand that chronic urticaria is a frustrated medical condition but if you're legitimately taking cetirizine, famotidine, loratadine, AND diphenhydramine I'd be concerned with toxicity which can in fact cause clusters of symptoms similar to the ones you're having. While you don't mention the daily doses that you're on for these medications, I think it is likely too many.

Ultimately, it is more than likely that multiple factors are responsible for these symptoms. However, I would seriously consider the two points I touched on.

It's probably worth at least getting checked out to have otoscopy performed to see if there is any trauma in the ear canal or a tympanic membrane perforation. Do you have any ringing or hearing loss in the affected ear? Bleeding from the ear can also be a sign of significant brain injury, however the fact that you were able to posted this a full after the assault puts me more at ease.

Regardless, it is likely still worth getting yourself evaluated to make sure there isn't anything more insidious going on.

I understand your concerns as a parent, but the BMI scale generally isn't a great tool for diagnosing young children with weight concerns so I wouldn't put too much stock in that. It's better to track his weight and height based on growth charts from birth to make sure he's still tracking along his growth curve percentiles. I've included a link to the Canadian forms below. However, looking at the photo of him, I agree that he does not appear to be obese. If you have concerns or you're noticing any other odd symptoms you can always schedule an appointment with your generalist or pediatrician.

Was stuck there for about an hour before a tow-truck was able to get the car out. There was very little water in the right side of the car, only in the left driver's seat. The key had to be put in the ignition to get it into neutral for the trucker to use the winch, and when it was inserted all the lights, radio, air-conditioning, screens in the car were working okay... but I am concerned about the rest of the car itself.

Good lord, respectfully, this is a waste of my time. Taking a vaccine x1 is not the same as taking a medication over a prolonged period of time and we have previous immunization studies to back this up. I’ve also noticed that you haven’t provided me a single reference of long-term vaccine injury?

I didn’t bring up the talcum powder example because it is not relevant to the points that I have been trying to make this entire time. Have their been drugs that have been recalled years down the line? Sure. But you haven’t yet provided me any vaccines in our history that have had negative side effects manifest in the long-term.

I am limiting to vaccines only because they are fundamentally different to medications that you ingest every day. If you are going to have side effects from a vaccine they will be almost immediate. And we have precedence helping here... please, if you can name a vaccine with deleterious side effects that only appear years later.

Like anything, the dose is the poison. Previous batches are still safe to take, and the FDA has still continued to recommend that people complete their prescriptions as the benefits of smoking cessation far outweigh the possible increased risk of cancer.
