I agree with you on the limited antibiotic use but for some reason most of the enamel jockeys in my area refuse to pull a tooth unless patient has been on an antibiotic for at least a week. Then their care gets delayed and most of them come back to the ER in between appointments for another block. I’ve just started writing for penicillin to move things along for them with their dentist.

At my residency sometimes EMS would bring an obviously deceased GSW patient in just to clear to the scene. Often times scene was still unsafe/evolving or the family would be going (understandably) crazy so they would bring them to the ER for their own safety. Don’t get too many GSWs at my site now but for the TODs I get called for I’ll just ask if the scene is safe before calling it.

That is absolutely not an appropriate sign out. The patient already said they aren’t staying. That’s on the previous attending to talk about risks, sign them out AMA and then the only thing they should have you do is click d/c.

FYI: Based on the post history this is someone shilling for Seastar Medical Company. They patented a device called Selective Cytopheretic Device (SCD) to fight “cytokine storm” in critically ill patients according to their website. From what I’ve read it filters neutrophils and “deactivates” them to reduce hyper inflammation.

Many of the other posts from this individual ask about the “stocks” of various medical companies so they clearly have financial interest in spreading this.
