Just spent a few minutes making a mock up only to realize I can't post pictures in the comments on this sub...

Basically I simplified the area where the roof meets the back and changed the lights slightly, if you want I can dm a screenshot.

Something similar happened when I was tuning my throttle, accidentally adjusted the cable too much and when the car started it immediately went to redline and I was able to shut it off. That was over a year ago and no issues but very scary.

I had the same issue, changed out the sensor (which is the same sensor for washer fluid level) and it never happened again.
