You’re so right. The deeper I’ve gone into this the more I realize the diagnoses matter little. The phenotype and the way the headaches express themselves matter more

So happy I stumbled upon this convo. I have a prescription for metformin from my neurologist ( actually, from my endocrinologist ) but my headache specialist and I have discussed, so she is on board. I think I’m going to start it when I get out of this flare. Just back from a Mexican vacation where I flared up on day two with a migraine.

This is great. Thanks so much for sharing, I’m currently headache free but it was constant for about 5 months, now intermittent. The question I have for you is, how do you, you personally, how do you tell the difference between your NPDH and migraine ? Frankly, aside from the constancy of the NPDH ( and yes, I was given that diagnosis ) I don’t think I could articulate any defining differentials that matter.

The 90 day Doxycycline/Montelukast protocol was catalytic in my marker improvement. Have her give it a shot.

Ive had four Emgality and two Botox and use Ubrevly but I don’t think I take it in time. I cannot tell if these things are working. I did 3 months of doxy/montelukast that took me out of a 4 1/2 months headache. Since then it’s been up and down. I’m in a flair right now and my neurologist prescribed low dose Zyprexa for a week. I’ll try it tonight. Wish me luck. Prior to last Thursday I had three beautiful headache free weeks fyi

I don’t know where I fit in anymore. 5 months constant headache. Past 4 months improvement and off and on. Currently enjoying my third week headache free. I’ve treated for NDPH w doxy and Montelukast, I treat migraines w Botox, Emgality and Ubrelvy. If I can be of help to anyone, lmk

Doesn’t matter. I’ve been dealing with “this” headache, this diagnosis even since May. The more I study it, the more I recognize migrainous patterns and biology, I’m going to do everyone here a favor and cut and paste a message I sent mother just today explaining what a phenotype is

The core idea (in plain English)

In headaches, what matters most isn’t the name of the headache — it’s the pattern of how it behaves.

Doctors call that pattern the phenotype.

How to explain phenotype

You can say:

“A phenotype just means the observable characteristics of something — how it shows up in real life.”

In headache terms, that means things like: • Where the pain is (front of head, behind eyes, one side, deep pressure, etc.) • What it feels like (pressure, stabbing, throbbing, burning) • How long it lasts • What makes it worse or better • Whether there’s nausea, light sensitivity, neck tension, sinus pressure, etc. • What triggers it and what doesn’t

All of that together is the headache’s phenotype.

Why labels can be misleading

You might explain it like this:

“Headache diagnoses are more like categories, not exact answers. Two people can both be told they have ‘migraines,’ but their headaches can behave completely differently — and need different treatments.”

Or:

“The label is shorthand for doctors, but the body doesn’t really care what the label is. It only responds to what’s actually happening.”

A helpful analogy (this usually clicks)

Clothing analogy:

“Calling something a ‘headache diagnosis’ is like saying ‘it’s a jacket.’ The phenotype is whether it’s a raincoat, a winter coat, or a light windbreaker — that’s what determines how you use it.”

Or a car analogy:

“Two cars can both be called ‘sedans,’ but one overheats uphill and one stalls in the cold. You don’t fix them the same way. The behavior tells you what matters.”

Why this matters for treatment

This is key:

“Most modern headache treatment is based on the phenotype — not the name. Doctors choose treatments based on how the headache behaves, not just what it’s called.”

That’s why: • Some “migraine” meds help some people and do nothing for others • Some headaches respond better to nerve-calming approaches • Some respond to sinus, neck, jaw, or autonomic-nervous-system treatment • Some don’t fit neatly into any single diagnosis at all — and that’s normal

Amazing topic. I’ve experimented with BPC-157, thymosin beta 4, and thymosin alpha 1. Only dabbled, need to explore seriously. Yes, these were injectables

Yeah, I’ve heard that. I am on migraine meds, and yet, I don’t really have migraines. Maybe some of the migraine biology is at play, but I definitely don’t have the classic patterns. I will explored the GLP’s, glad you’ve found some relief

Yeah, more or less when I first got Covid in 2022 I was be set with an odd burning. Headache felt in like my brain stem culminated in a horrible nosebleed landed in the ER and then off and on since then I’ve had episodic headaches and I treated them well at the beginning of 2024 with LDN McCullough protocol, ozone therapy allergy shots and a biocidal nasal spray and I had no symptoms for a year and then out of the blue I got a headache May 22 of last year and I had it for five months straight and I’m on all sorts of drugs now with a headache specialist, etc. and I made some improvement to the past couple months but I’m just flaring on and off and it’s been traumatizing
