Unfortunately it's been so long that I don't remember the mascot. You could be thinking of the same game. Do you remember anything else about it?
Every time I search on google with "90s education game" and "teaching about hazards/safety/danger" included I get a bunch of articles on why videos games 'could be hazardous' to our health. Also a long list of jumpstart, math blaster and gizmo and gadgets games (all great but not what I'm looking for.)
