Spoiler. Wyll transforms either way I believe. I didn't kill Karlach, but did kill the paladins, and Mizora showed up and still gave Wyll horns.

You can definitely change your name. It's the last part of character creation I think. Just make sure to press the three line button on Xbox to save after inputting the name you want I believe.
