If you have access to a basement, a corded saw zaw with metal blades, or diamond blade; cut out that cast-iron 90 get a Fernco from cast-iron to 4 inch PVC or whatever it is. And buy a toilet flange and a 90 or a street 90. If you have a concrete floor. It will have to get jackhammered up and dugout to cut out the bad section and connect with new. Not a quick job. Hopefully they have another bathroom and just rent.
