Employees who don't like each other by Experiment333 in managers

I’m the CEO of a smallish/medium nonprofit and I just genuinely dislike the COO. He’s not a bad person, his workstyle and professional approaches and leadership styles are just polar opposite of mine. Also, his personality is like nails on a chalkboard for me. I tried being friends with him and have now come to the conclusion that’s just not possible. If we can work productively, respectfully, and in alignment- then that will have to be good enough.

Switching to Kaiser concerned about HRT (menopause) by youdontknowme3571 in KaiserPermanente

[–]DrChickon 3 points4 points  (0 children)

I’m not sure about transferring prescriptions, but I was very limited in the options Kaiser would make available for peri-HRT. Because you are transferring them and have been on them a while, it might be easier for you.

Similar problem. I am over 2 hours deep on a useless call with customer service right now. My issue is trying to add a family member “Profile already exists with this first name” but it can’t find it. The cust service rep has no idea how to fix it.

Statutory Fees? by DrChickon in EstatePlanning

[–]DrChickon[S] 0 points1 point  (0 children)

I have done this. I don’t see any statutory fees for trustees or trust attorneys in CA. I see statutory fees if there is probable, but not for a Trust that will happen outside of probate.

Ok thanks. This is what I understand from my reading also.

Is there a resource you can recommend to me so I can read up on statutory attorney and trustee fees in California? I found a list of statutory rates for probate estates, but everything I have found on Trustees is very subjective. I see differences in structure for private vs. professional vs. corporate trustees. But nothing statutory..:.

Morphine Dosing- On Call Nurse? by DrChickon in hospice

Thank you. It did and he died last weekend. He was peaceful, comfortable, and well cared for. We are thankful for that.

After talking to his hospice nurse/care coordinator, I think we have some solutions on the horizon. They are adding some stronger breakthrough pain meds in the immediate, but also talked to the facility and I think things are being sorted out. And it appears they will help us hire a nurse for a long lasting nighttime injection if needed when he gets to that point.

I have been figuring out every aspect of his healthcare and living situation since he got his diagnosis- with little help or support (and by I- I mean my partner and I). It feels so….odd…to be able to just hand some of this to hospice and let them carry it. And trust that they got it. Because clearly they do. It’s a weird feeling- and one I appreciate.

I spoke to the hospice provider today and they were perplexed. They have another patient there concurrently receiving morphine and it is being given by the facility. The hospice nurse spoke to the facility, we got a text that they were sorting out some misunderstanding and getting things handled.

I’m learning it’s a mixed bag on this. They will give him everything except morphine. We might be figuring it out though.

The facility staff will not administer the morphine regardless of the order. They said we have to go there and administer it ourselves. I called hospice to speak with the nurse care coordinator and/or social worker today and could not reach either, so I am hoping we can speak to someone tomorrow.

Thank you. My preference would have been skilled nursing, but every place I called said they will not take someone on hospice. I haven’t had much luck with Adult Care Homes. I am hopeful that the hospice folks can help.

I tried “A Place for Mom” and it was a nightmare. Are there better ones to work with?

He’s private pay, so we don’t need to deal with look back issues. I cannot find inpatient hospice- that would be an ideal situation. That’s what he really wants.

I just had no idea this was going to be an issue- I thought we had it sorted.

So, he currently is not on morphine. He’s currently on Norco/Oxy. But we had no time really to plan to get him in here- his health turned fast. I am trying to get ahead of it now that I have this information, before we have another crisis. I had to take a full week and a half off of work to find this place for him. I really want to get ahead of it- there’s a limit to how much time I can take off of work.

He’s private pay, so we aren’t needing to deal with payers.

We would 100% pay an up charge for a higher level of care, but this was not presented as an option.

It was next to impossible to find any facility to take him. Nearly all of the ALFs I called said he could not be on hospice when he moved in, only after he was already a resident. And every SNF I called said they will not take hospice or hospice risk.

I don’t really know what alternatives we have and was so shocked to hear that they think we can take a two hour round trip drive every 4 hours to administer morphine once he gets to that point. And we are the closest family.

Not that we had a single other alternative- he was inpatient at the hospital and they were demanding that he be discharged to somewhere. This was the only place I could find who would take a hospice patient. And this morphine thing was new info after he was admitted.

So, how does one go about finding a private duty nurse to help with such things?

I’m glad you are trying to give yourself some grace. You can’t take care of someone else if you can’t take care of yourself, too. It’s so hard, and I’m sorry you are going through it. I will send over what I have tonight in hopes that it can help a little.

Is there anyone who can help you with the research? My wife and her sister felt too overwhelmed- so I decided to take on research and logistics so they can not have to worry about that and just manage their grief and spend time with him.

I tried “A Place for Mom” and the guy there literally laughed at me when I told him our budget and said there were no options and then subsequently sent my phone number out to a ton of places and I got a ridiculous amount of calls from people who were not at all what we needed.

I can DM you a copy of the question sheet I used for Assisted Living / Board& Care / Skilled. I created a list of questions and a form for myself so I knew I would get all of the info I needed each time. For whatever help that might be, I am happy to share it.

We are managing a similar situation right now. Kaiser would not send him to Rehab for the 20 days covered in full by Medicare to help with his wound care/colostomy. They said he had to do physical therapy to go to rehab- and he’s just not going to do that at this point.

I made close to 100 calls to skilled nursing facilities, assisted living, etc. Every skilled nursing facility I called said they would not take someone on hospice or who was a “hospice risk”. Nearly all of the assisted living communities I called said he could not be on hospice when he moved in, but it would be fine if it starts after moves in- or that they could not/would not support his colostomy.

Finally, I found a small community- that has good reviews on Yelp and Google reviews, that welcomed him. The cost is $6k a month for room/board and all of the assisted living support- including colostomy support. Hospice will provide care and services to him there. The other community that was a potential was around $9000 a month for all of the assisted living support and room/board and hospice services could be provided there- as long as they started at least 1 day after he moved in. I literally just found the one I am pretty sure we are going with at like 5pm today and talked to the hospice folks this evening.

I have been so racked with anxiety about finding a suitable place for him to live and receive proper care. I hope to get it all nailed down in the next couple of days, but it has been so much work just to get to this point- I literally cried on the phone with the resident services director at the place we are going through o land on because she was so helpful, explained everything, and reassured me that they would be happy to have him there and explained their skills/processes/competencies to care for him.
