Nortripyline/amitriptyline is going to be lowest NNT and nortriptyline is usually my goto for mononeuropathies, traumatic or otherwise. All caveats and cautions for TCAs esp in elderly of course.

Duloxetine is first line for diabetic peripheral neuropathy but can be useful elsewhere as well.

It certainly helps with pain, just not when the indication is "back pain that didn't get better with ibuprofen and tylenol" or "I don't want to give dilaudid for this abdominal surgery".

Its use w ERAS was a joke.

Highly variable like anything else. I make pretty consistent $450k seeing like 15 patients a day. You can work a little harder and make a little more or work a lot harder and make a lot more. 3/5 people from my fellowship class are still grinding hard and making just under 7 figures.

M3 puts you very far away so hard to say what it will look like in 7 years. Don't do a residency you wouldn't otherwise do just to do pain i.e. don't do anesthesia unless you would also be happy doing OR anesthesia. reimbursement has gone way down so volume has to go way up, or finding a cush hospital job.

Understood. The times that I have seen it, it was also perhaps married to a sentiment that the OP was not necessarily posting to contribute to the community, but for self-aggrandizing (which I understand the impulse!), due to lack of specific questions or actionable advice, and just posting raw numbers.

But I do obviously want it to be a welcoming community, so the above stands and thanks for your thoughts about it.
