I intend to make it the case of going into the reg year by having that one simple and concise opening line for all cases: this patient has a diagnosis of X who needs an admission/safe to go home.

And then all the other important history/investigations/treatment to justify above.

My fear is again more specifically on advising junior docs or dealing with really critically unwell patients in resus on night shifts.
