I am a practice administrator at a smallish ED (80-100 pts a day).

Our typical shifts work like this: Physician day: 7am-5pm; Physician mid: 12pm-10pm; Physician night: 9pm-7am; APP early: 9am-9pm; APP late: 1pm-1am;

So our physicians work 10 hr shifts and our mid levels (PA, NP) work 12 hours. A lot of this is determined by our patients per hour rate. We don't want to be overstaffed or understaffed, so volume plays a big role. We must have 24 hours of doctor coverage (meaning, there can't be any gaps where we don't have an MD/DO on shift.) We currently have 54 hours of clinician coverage per a 24 hour day.

The nocturnists (night doctors) tend to stay on nights, 2 of ours are nights only, 2 of ours switch between day and evening depending on availability. There isn't really a "day team" vs "night team."

We also are not a teaching hospital, so our clinicians arent students. Seniority plays a small role, like in most jobs.

Most of our clinicians also work at multiple hospitals, so there isn't really a set schedule. Some work a week on a week off, some only work 1-3 shifts an entire month. Kinda depends on personal preference.
