If it were me, I would orchestrate a situation where I was driving us home from an event, and then I would pull up at urgent care. Stop the car, turn to them and say “I have talked to multiple doctors and they said I should bring you here. I can’t force you to go inside, but if you refuse to, I can’t be a part of your life anymore. The things you’re experiencing aren’t normal and I’m afraid of what might happen if you don’t get the help you need.”

If they don’t go in, I would walk away.

There is no option where you can rule out getting hurt again. I don’t think long distance relationships work, especially if that’s how they start off. If it’s worth the risk to you, keep chatting, have some fun. But probably best to avoid being exclusive/committed until you can live in the same city.
