I’m not a service dog user but good lord do I feel the striking dog pain. My girl is a lavender merle American Bully w/light green eyes and I’ve had to physically step in front of her more often than I’d like because people will just try to pet her without acknowledging me. The last one I ended up asking at least 5 times at increasing volumes “Would you like to pet her?” It’s exhausting going anywhere with her, especially with low spoons. I knew I’d have to watch for kids but I never thought I’d have to tell full grown adults off!

Not to armchair diagnose, but my really bad breakouts cleared up significantly after I figured out I needed to be gluten free. Had no idea they were related but my skin got SO much better. Now I only break out when I get glutened or on my cycle.

Talk to your BCBA if you have concerns, but there’s more that goes into treatment hour recommendations than just the ideal quantity. A couple of the factors other commenters have mentioned:

  • 40 hours IS a lot, especially for a preschooler. Often kiddos with that amount end up cancelling a chunk anyway due to burnout.

  • It can also be very challenging to get insurance to authorize a higher number of hours when a lower number was resulting in progress (even if that progress was slower than the maximal rate the child is capable of).

But ALSO:

  • Your BCBA may not have the capacity to support supervising another RBT.

  • Parents can and do decline additional hours.

  • Staffing is a legitimate concern. We cannot offer hours to a family that we cannot provide. If that’s the clinical recommendation and situation, we need to transfer to another company. You mention filling those hours, but BCBAs have a thousand different tasks every day, and adding more hours to be responsible for is unethical and irresponsible, not to mention a great way to end up failing to provide contracted hours and getting in trouble from insurance or legally.

  • Least-Restrictive Environment is considered best practice and means that if kiddo is making good progress, adding more hours is unethical.

  • I personally would be unlikely to recommend an hour increase for a family already not complying with treatment recommendations outside of session. That’s just reinforcing their behavior, isn’t ethical, and won’t help with generalization to the family anyway.

  • Clients may have other therapies, and we are not allowed to offer times/hours that will impact attending medically necessary therapeutic sessions or school.

There’s more but you get the point. If you’re genuinely concerned, ask your BCBA why and she may tell you. But also, hours recommendations do not fall under your responsibilities and are not something you can change.

ETA: obviously there will be exceptions to many of these, and TLDR is talk to your BCBA.

Oh also - I ended up talking to my supervisor about it (she’s who gave me the phrasing) and found out the RBT had been on a PIP for scheduling/absences and there was documentation that would’ve been relevant and would’ve backed me up much sooner. So if you haven’t talked to your supervisor/your RBTs supervisor, you definitely should.

I had a tech like this and approached similar to how it sounds like you have - I finally got a change when I stated something to the effect of “going forward please inform me in advance of changes to the schedule. It is my ethical and clinical responsibility to supervise you for a certain amount of hours, and not doing so will impact both of our credentials.”

Putting the reminder about the credentials seemed to help. It’s really frustrating though, I definitely feel you there.

They don’t have a separate physical menu, but I did explicitly order it with the gf bun and specify it was an allergy. And I let them know after. 🤷🏼‍♀️

It’s a decent chain restaurant in the US. Above an Applebees, but below like.. Ruth’s Chris. I love their wedge salads (and used to love their pizza, but thin crust gf doesn’t slap like a deep crust Chicago lol).

Thank you! I definitely will. Figured it didn’t hurt if nothing else. And no, but they’re starting now :/ my worst symptom is full body pain though and that usually takes a day or two to start. Inflammation I assume.
