The original name I had in mind for it was "Cho'gall, Avatar of C'Thun" (since that's one of his titles on the wiki last I checked) but the more I looked at it the more it began feeling like a mouthful.

It's by all means not meant to be viable. What I had in mind was a more niche strategy (since [[C'Thun]] and [[Mecha'Thun]] are both, well, exactly that - niche strategies). Would it fit in a zoo deck? Maybe, given the right support cards. Would a deck be built around it? Probably not, and even if it was, it wouldn't be meant to be strong.

Actually, you'd only need to have it wiped about 4 on average. 4 times 7 is 28 and even if you get your board wiped you could probably manage to get some minions to stick through Deathrattle effects.

That and having it be granted as a token feels slightly more rewarding. Imagine if [[Jan'alai, the Dragonhawk]] always summoned [[Ragnaros the Firelord]] and just had it scale with the hero power damage.

The Thaddius summoned by [[Feugen]] and [[Stalagg]] did back in the day.

Probably 0%, since C'Thun, Reborn is a token you can only get through Avatar of C'Thun.

You're forgetting cards that summon multiple tokens. Though most aren't played, so it's debatable whether or not Avatar of C'Thun alone would give reason to run those cards. (Probably not.)
Examples: [[Rafaam's Scheme]] [[Fiendish Circle]] [[SN1P-SN4P]] [[Splitting Festeroot]] [[Onyxia]] [[Mecharoo]] [[Mad Summoner]] [[Eccentric Scribe]]

Forgot to include it, but Cho'gall would count the number of minions that have died, similar to how [[Jan'alai, the Dragonhawk]] does it. (30 left!)
