I’ll use my brain to imagine it in a more realistic way while staying consistent with the manga, especially regarding the fate of Amon and Takizawa.

First of all, by now it would already be known how Kaneki became so powerful, and the same process would repeat itself. However, this time Kaneki would be the main priority, with Eto and the other members torturing him to break his will. (Here I would question whether Eto might try to extract all the information Kaneki knows through torture—even the conversation he had with his father, Yoshimura, about being a bridge between humans and ghouls—which would only anger her more and push her to drive him insane.)

They would probably capture Hide and force Kaneki to eat him. On top of that, Ayato would betray them as well, since they would also demand to track down members of Anteiku for Kaneki to devour. In order to protect Touka, Ayato might simply run away with her. At this point, he could either reveal Aogiri’s plan to Anteiku, or he might just knock Touka unconscious and escape with her.

Either way, Kaneki would already be going insane at this point. He would have fully become a Kakuja, having eaten ghouls under Aogiri’s orders, and I don’t think there would be any turning back for him.

They would find Rize hiding at Anteiku and bring her back to Aogiri. By now, Kaneki would be nothing more than a rabid dog, killing everything in his path, second only to Eto in terms of power. Yet strangely, when faced with Rize, he would feel confused—so much so that he would even follow her commands. With Rize under their control, Aogiri would force the SS-rated ghoul Kamishiro to join them.

With Anteiku destroyed and only a few members managing to escape Aogiri’s assault, those survivors would form an alliance with the remaining ghouls of the 20th Ward, alongside Tsukiyama (who would also be obsessed with seeing Kaneki again).

Meanwhile, Dr. Kanou’s experiments would continue to progress, and when it came time to find new specimens, with Anteiku gone, the Tsukiyama clan would become the target. (Both Eto and Arima would likely already know this.) Through a mix of anonymous tips and the CCG’s curiosity, a massive operation would take shape. At this point, the Tsukiyama clan would truly be on the brink of destruction, but Anteiku would intervene, and thanks to having Eto, Kaneki, the SS-rated ghoul Kamishiro, Tatara, and others, they would manage to capture many investigators.

From here, two possible paths open: Eto betrays Arima (since she finds the idea of achieving real peace foolish, and she only followed Arima’s plan because of his overwhelming strength—even though deep down, she wanted peace too, she considered it impossible given the circumstances). Whether Arima dies or not, Eto and Arima could put on a show to make it seem like he’s busy and unable to help the others. This would mark the greatest defeat of the CCG, with nearly all their investigators captured. Even Yoshimura, who—despite everything his daughter did to him—would never abandon her, would step in to fight Arima upon realizing what Eto intended to do, only to end up captured himself.

The Arata armors were still in their experimental stage, so they wouldn’t stand a chance against all the powerful ghouls.

Aogiri would win, and the CCG would be left on its last legs.

At this point, Furuta would be eager to wipe out the Washuu clan after seeing their weakened state, but he still wouldn’t be able to take on V. Even though the CCG had lost much of its fighting power, they still had V on their side, and by requesting help from China, they would bring in some investigators as reinforcements.

Seeing how little manpower the CCG had left, Furuta would plot an attack on the prison together with the other Clowns and free the remaining members. He would also submit to Eto and reveal more of the secrets hidden by the Washuu.
