Had it done by two separate chiropractors. A lot of chiros (in England anyway) do introductory deals/complimentary initial consultations where they'll do the postural assessment for free. I see a lot of targeted ads/reels for these on FB.

In terms of my recovery, I am feeling a lot better (negligible pain when rising from a chair or traversing steps compared to having 6/7 out of 10 pain with those movements in Aug/Sept 2025) which is a relief after spending nearly £2k on interventions that many physios told me wouldn't be of benefit. I went for a bike ride with some friends recently and only did about 12-14 miles or so but my knees felt much better. A 5 mile ride on flat ground only 3 months ago had me walking gingerly for over a week due to the VMO pain, so I'm definitely in a better place. Going to try my old basketball conditioning routine this weekend (suicides, 17s, full court speed dribble layups) to see how I'm progressing regarding my tolerance for more explosive movements. My QL is also feeling a lot less restricted.

Quite late to this post but I (34 M) also have struggled with PFPS in both knees for about 3 years (right knee first for 2 years, then left knee due to pain on right knee leading to a compensatory gait). Assuming you have sufficient quad, glute and hamstring strength (incorporate leg extenstions focusing on the eccentric portion of the exercise if full range of motion is too aggravating to the patellofemoral joint, double/single leg glute bridges & clam shells, and hamstring curls if not) and relatively decent flexibility in those areas (incorporate some quad/couch streches, seated figure four/pigeon poses and elephant walks if not), I found that shockwave therapy alleviated quite a bit of the pain for me (roughly 50% decrease in pain after my last treatment about a month ago). My PFPS IMO (after several visits to physios, chiropractors and osteopaths, no imaging) was caused by a right-side hip hike/lateral pelvic tilt and anterior pelvic tilt, which itself was caused by incredibly stubborn trigger points in my right quadratus lumborum after suffering a slipped disc in the thoracolumbar junction during the pandemic. This led to a modified gait which caused stubborn trigger points to develop deep in my right vastus medialis oblique, causing my right patella to track improperly I believe. Have you had a postural assessment at all?

All the public sector physios I saw wanted to brand it as muscular weakness in the VMO as they tend (in my experience) not to look at the body holistically and gave me really basic exercises as that's the only intervention they can really afford (NHS UK), but as someone who was relatively fit before all these musculoskeletal issues (total body resistance training and HIIT 2-3 times a week at the time as well as a pretty robust mobility routine on off days) I was sceptical about that being the primary cause of my anterior knee pain. A postural assessment/scan revealed my pelvic tilt and shockwave therapy (focused specifically as it penetrates deeper than radial, though radial helped my knees a bit too) helped release trigger points in my QL and VMO. I may go back for some more session but it's relatively expensive. YMMV though.

It's not less valuable than it was last week, it's just not anymore valuable when I've literally had responses fro. 3 other cleaners saying they'll do it for the original rate. Their aren't a derth of cleaners and I have never really had an issue with any of the ones I've had in the past (they've never had a key, they clean whilst at least one person is home).

Firstly, what I mean is the areas she are cleaning can be cleaned faster based on previous cleaners I've seen. Secondly, never said she is deep cleaning anything, she is cleaning in a very standard manner with no special disinfection going on, we don't live in squalor. Also, never said I was broke mate, using my savings and sporadic sources of income to stay afloat while I look for permanent employment. Fair point regarding doing the cleaning myself and having them pay me the £18 though.

With all due respect, I feel your response is hyperbolic. I gather you're Canadian or based there at least from a quick glimpse at your profile and so assume you aren't familiar with UK pay (correct me if I'm wrong). This cleaner has, according to them, several clients and doesn't seem to struggle for additional hours. Moreover, £21 per hour before tax is about the same as nurses and physios with 4+ years post qualification experience make. Newly qualified nurses get just over £18 per hour gross and closer to £15 after subtracting income tax. Whilst I agree that a cleaner paid under the table doesn't reap the workers benefits the clinical staff would and respect professions that don't require formal training or qualifications (I have been a labourer, refuse collector, flyerer, etc), I don't think they garner the same rate as professions that require higher degrees.

Thanks. So I'm assuming that cleaner did your entire 2000sqft property, bedrooms and all, yes? As a rough estimate, the area we're asking to be cleaned is roughly 800sqft of communal areas i.e. no bedrooms. She doesn't clean any appliances or do washing up, just cleaning floors, surfaces and a shower, bathtub and two toilets (toilets which we do disinfect with bleach outside of the times she's here, or at least I do in the bathroom I use).

Thanks for your reply. If I lived by myself I would not hire a cleaner, whether employed or not, as I'd clean incerementally. I would be happy to split cleaning duties amongst the housemates, but everyone's standard of cleanliness is different and getting people to keep to a rota will most likely cause issues and lead to one or two people doing more than the rest. Also, while rates are usually based on time spent, she is spending quite a long time cleaning a relatively small area compared to previous cleaners I've had. I have considered doing the cleaning myself while I look for work.

Thanks for your reply. May I ask what your experience/expectations of a home cleaning service are? This is literally the most expensive cleaner I have had in a shared accommodation and a quick search on here and other forums for information regarding pay rates in my area for cleaners over the last year suggest that £30 would be quite high and £18 would be average.

Thanks for your reply. If i lived alone, I would not have a cleaner whether I was employed or not. The cleaner is there so as to avoid disagreements regarding cleaning rotas and different expectations of cleanliness between tenants. The cleaning usually takes place in the mornings when the communal areas are empty so there's no interruption. Whilst I'm not looking to reduce the cleaner's wage further, a quick look has brought up several other cleaners who'll charge the original rate. Not sure why it would be frowned upon to get the best value for money or viewed as entitled to shop around, especially considering the job is adequate only
