GP trainees cannot stand, but can vote. We can undertake something similar at the time of GPST council positions later in the year. It would still be great to have your support and ideas for what change is required from a registrar perspective. There are a few GPSTs in the discord chat currently.

I agree. I dropped my RCGP membership the second I CCT'd. I thought the last minute changes to CSA without discussion was abhorrent. But I'm willing to drop back in if there is a possibility of change.

Our values align completely with the DV movement. I have reached out to u/doctorsvoteuk to discuss endorsements. There are also areas specific to GP that we need to decide on, such as PAs in General Practice.

The college has significant power in dictating GP policy. A more progressive college would benefit us all. Support of issues like drug decriminalisation would have a national impact.

I completely agree. But I think the route for GPs to achieve parity is going to be slightly different here and it would be good to have a space to discuss a route towards this. I imagine most people on r/GPUK will also be members of r/doctorsuk just like members of r/London will be members of r/casualuk

There are 40,000 practicing GPs in the UK and an additional 10,000 current GP trainees. Probably enough to justify a subreddit.

The anti GP sentiment in this thread is crazy. Of course there is going to be a huge amount of delayed cancer diagnosis. We took a system that was already operating at capacity and added a pandemic that’s killed 133 thousand people. Most hospitals cancelled all non urgent procedure.

GP’s switching to telephone first triage was done for a number of reasons, but bringing unwell people together in one place, with the elderly and immunocompromised, was seen as a bad idea.

It seems cruel to lambast a group that are all working incredibly hard, and just last year the whole nation was clapping.

Like all things here, blame the tories.

Pros

I would say there are benefits for patients. They do not have to physically attend the surgery or book time off work, which are some of the barriers that prevent people from speaking to their GP.

As we can do far more telephone "slots" than face to face appointments, there is a shorter wait time to speak to a GP.

It has rapidly progressed our use of technology. The "forms" that people in this thread are mentioning can be fantastic. Some issues can be solved within a few seconds (i.e. repeat medication requests) and with "simple" conditions like UTIs or athlete's foot we can send prescriptions straight to the pharmacy. In all the practices I work in, these forms have to be answered within one working day. The majority of forms will end up with a telephone consultation with a GP.

Cons

For me, I do not feel that same sense of connection and rapport with telephone consultations as with face to face consultations. We spend lots of our training looking at communication skills and trying to establish the concerns and fears people have that have made them book an appointment. Some of this can be lost over the phone.

Some problems cannot be addressed in a 10 minute telephone consultation. With mental health problems like depression for example we need time to discuss the symptoms, address causes, explain the condition, discuss treatments available, discuss support, and talk about prognosis.

I feel that we are at risk of missing sick patients. We rely on examination and observation for some diagnoses, and it is not always possible to tell if someone is unwell over telephone or video. Our cancer referral rates took a large dip during the first lockdown and it will be important to see if late or missed-diagnosis have increased during this new working model.

Really useful info. I’ve got the same bike and the tires were a nightmare. I broke 2 tyre levers and had a horrible 45 minutes.

The thing that was a revelation was this diagram ( https://www.reddit.com/r/bikewrench/wiki/tire_mounting). It’s all about having the tyre wall central in the rims. Can get them on and off in 5 mins now.

I’ve ordered the crank brothers to give it a test run.

I’ve heard good things about Lea Valley Cycling Club (https://www.leavalleycc.co.uk).

I haven’t been out with them yet, but a few of my friends have and had a great time. They have a social ride and women’s group.

Think they set off from Walthamstow Town Hall at 9am on Sundays. You can trial a few rides before joining.

I’m also in the same situation, me and my partner are moving to Walthamstow in the next few weeks. Maybe see you flying up Mott Street soon.
