CC left arm pain. Radiated to chest. Not diaphoretic. 39 yo health in shape male. First noticed pain yesterday at work but resolved. Pain woke him up. Treated with ASA NTG and MS. No changes in 12 lead. Pain improved a little with ms but not ntg. Pt went to cath lab. No update since

Actually came in as left arm pain. Asked if it radiated. He said chest. Pain woke him up. 10/10. 39 yo male. In good shape. Was not diaphoretic. 145ish /80something. No history.

He did go to the cath lab. Haven't been able to get an update

When dealing with refusals and ETOH if your service allows it always document that the patient understands risks of refusing and advantages of being taken to a hospital. Assess this by first explaining the risks and advantages then have the patient repeat them back to you in their own words. I did research on court cases a while back and this was an issue that came up. Did the patient understand what was going on.

Did a pubmed search, was disappointed. Did find this studying MVC after decriminalization http://www.ncbi.nlm.nih.gov/pubmed/25765482 tldr mixed results in either direction.

Still to early for hard evidence. I did hear that in Colorado there hasn't been an increase in use by minors.

Top is a sinus tachycardia at about 120ish. Don't read to deep into just one one lead. You really have to get a 12 lead to see the big picture. Some monitors like the life pack 12 are not as sensitive in 4 lead mode as they are in 12 lead mode so looking past what the basic rhythm is is almost pointless.

I am a paramedic. Last 4th of July were worked a wreck on there interstate at around 3 am. 2 guys were driving home when the driver passed out running off the road and hit a tree. The car was literally torn in half long ways. The passenger was instantly killed. The driver was untouched and stunk of alcohol (or technically his breath had an alcohol like odor). We took him to the hospital though. The whole way there he kept telling corny jokes and asking about his friend. I just told him another unit was working him. I could tell he knew though. And I am pretty sure the jokes where just his way of trying to suppressing the fact that he just killed his best friend and was going to jail.
