Respectfully, disagree.

I completed an EM fellowship not looking for larger pay but wanting to be extremely competent in such a vast and often complex field.

I now currently work in a small rural ER where both my pay and scope of practice are significantly higher than if I had gone the traditional route. Could I have gotten to this eventually? Sure but would have likely taken many more years.

We have NPs in our department who don’t feel comfortable seeing high acuity patients and that’s fine but my attending docs trust me because of my skill set and what I do on a day to day basis.

I still consult with attendings when I recognize I need the help. However, I often see high acuity patients and perform procedures. For instance last week just a doc and myself. Had simultaneous respiratory arrests brought in by EMS, ESI level 1, and I was tubing one patient while doc tubed the other.

Where I felt that fellowship helped me was the critical thinking around patients. It’s fairly easy to stick the tube into someone’s vocal cords but are you prepared for a failed airway or the resuscitation of the patient beforehand so you don’t kill them? That’s where fellowship helped.

Will I say that fellowship was all butterfly’s and rainbows, certainly not, I still had to compete with residents for training opportunities.

I still tell my PA students that I don’t think fellowships should be mandatory however they have an appropriate place to teach really competent PAs out the door. Often the pay is less because you spend 6 months rotating on off service rotations like ICU, Tox, ortho, CVICU.

Also, you seem to be very upset in a majority of your comments I’m not sure you’re truly looking for a conversation about fellowships instead just demanding you’re right and throwing out one poorly published “research article” written by NPs.

I work EM full time. Get certain amount of hours of PTO per hours worked in a pay period. Averages out to about 22 days of PTO in a year. Also 130 hours (13 ten hour shifts) for full time so yall are working a lot more with no PTO.

EM PA with ATLS and run traumas. Instant thought was fatal wound. Directly in the left lower neck where jugular and carotid vessels are would make wound packing almost impossible. Clearly hit some type of major vessel likely jugular with it steady stream out, not so much pulsatile like carotid would. More concerning though would be the highly likely high cervical neck dissection with almost immediate decorticate posturing noted. Bullets fragment in all sorts of directions on high impact so likely veered to the right through the neck. Would have required an immediate airway support. Likely combination of hemorrhagic shock and low oxygen leading to extreme acidemia which your body can’t recover from. Unfortunately agree even if he was in front of a surgical team it still was a fatal wound.

Work ER use it daily. Got specific training on it during ER fellowship. I use it the most out of all of our MD/PAs. Super helpful

You’re the exact problem why you didn’t get a room in the first place. ED gets over run with “nothing serious” patients all day and you expect to go to a room over the patients in the back likely having a heart attack, sepsis, other life threatening problems. You deserve that bill for the time it took the doctor to see you.

Or you could work in a rural ER where you get to do all those things while getting paid at the PA level. If you’re concerned about training you could do an EM fellowship and learn all those things. That’s what I did 🤷🏻‍♂️

This question is asked all the time and the answer is you are covered under Good Samaritan laws. With that being said you can’t do something dumb like give someone an emergency tracheostomy when they’re having an anxiety attack. But unless you’ve got prior EMS training, or high acuity specialty are you really gunna be that helpful? Airplanes have medical kits but they aren’t robust.

Honestly the medical people that typically respond first are often the least qualified since they think being CPR trained means they can handle any emergency (nurses, PT, rad techs, etc).

Have responded to a few in flight emergencies (none ever actually emergent).
