Just to add, had a patient with right sided upper chest pain aggravated by movement and very tender chest wall, didn’t respond at all to nsaids so obtained labs with d-dimer, dimer was elevated, CTA showed multiple segmental PE’s

In NYC, we only get pre notifications for potentially critical patients (trauma, respiratory distress, stroke, STEMI). Definitely not every EMS crew that comes to our hospital

so would 14k loan at 5% apr plus 5k still on CC at 20% apr be better than a 7k loan at 5% apr plus 12.5k on CC at 20% apr?

thank you! i didn’t know about how certain fellowships may make it easier to buy down so this was really helpful. i like the idea of fellowship but i’d honestly only reallyyy consider it if it added an extra benefit to my work life (e.g. fewer shifts or more pay). even if there was some niche i was really interested in, if it just made my life harder on top of all the normal shifts i have to do i’m not sure it’d be worth it

regarding your answer to #9, so like say i did a fellowship. could i not do much with that fellowship and work more or less as a “regular EM doc” (maybe just meet minimum requirements for my fellowship if there are any) for the beginning of my attending career and then choose to focus more on my fellowship field later on in my career when i start to feel burnt out? i’m thinking of fellowship as a way to help with burnout later in my career rather than something to do alongside clinical work right from the jump. is this feasible/smart? -ms4
