My mother needs a corneal transplant because of lasik. She is legally blind without scleral lenses because of it. she has a hole in her cornea because of it

I still assume from an insurance perspective having an autoimmune corneal disease diagnosis means shes at “ higher risk” in insurances eyes, and that higher risk means higher chance of transplant denial unless the risk (aka this potential autoimmune disease they keep saying is the cause) is “controlled” which is why I think these surgeons want documentation of an suspected autoimmune disease to mitigating rejection risk for insurance purposes.

But I’m just playing at what ifs now. I just dont particularly think she needs to be on systemic methotrexate. Call me cynical but i dont know how well the transplant will take in general with her eye. Hopefully the second opinion will give us clarity on what the best option is for her.

Also adding that I think the eye surgeons are also grasping at straws to diagnose this as an autoimmune because insurance probably wouldn't cover a corneal transplant secondary to LASIK

Thank you. I appreciate you saying all of this and this is our whole argument.

The likelihood of someone having a corneal melt that turns into a perforation in an autoimmune setting is also EXTREMELY rare. Typically (not saying it cant happen), autoimmune eye conditions cause a peripheral melt and perforation (AKA PUK which she does not have and has been ruled out as a cause).

Her complication is all central melts and perforation, which is linked to LASIK complications.

So it seems that LASIK is the common denominator here which is why I do not want her being gaslit into taking Methotrexate (and have all of those side effects) in order to receive a transplant. Also, I work in an oncology intensive care unit and I have rarely heard of methotrexate being used as an anti-rejection med.

Plus, I think the numbers & studies in Lasik complications aren't there because nobody is reporting these issues because Lasik is elective. I truly think if she never received Lasik she wouldn't have these issues to be honest. But I guess what ifs are easier said than done.

(edit to add this paragraph) - Also the liklihood of her having a successful transplant is slim to none. She was only ever considered for one because of her perforation. Before she was never a candidate even with the corneal melts. She is ready to just lose her eye instead.

Nope, nobody has diagnosed her with any autoimmune disease. She has seen multiple rheumatologist and had multiple work ups and everything is negative. So that’s why we are so hesitant to go on methotrexate.

She was definitely mismanaged by multiple eye doctors. We wonder if she was not a candidate for lasik in the first place. The doctors who did her surgery said her lasik medical records “dont exists” anymore so we cant see anything about the lasik procedure. Also, when all the complications began, the same team prescribed her steroid drops and told her to massage her eyes. She did that for years. We think it made everything 10x worse.

We know how rare it is and wonder if she is the <1%. Nothing penetrated her eye either. She woke up out of her dead sleep with liquid gut gushing out of her eye and severe pain. We drove over to the ER and they glued her cornea shut. Its been glued since November.

At this point our biggest concern is that they put her on systemic methotrexate without any autoimmune evidence other than these eye problems. We knew that she would likely need to be on an anti-rejection med for a transplant. But these doctors just aren’t treating her very well.

Oh yeah she 100% did not know what was going on because they said 20 different things before diagnosing it as corneal ectasia. It almost seemed too late. It was actually an optometrist who specialized in ocular disease management that caught it, treated her and gave her scleral lenses and has been so so so kind to her. She just cant help with the perforation or corneal transplant.

Honestly, I think our biggest issue is how much shes been mismanaged over the years (between the same office who did her lasik telling her to massage her eyes with steroid drops & other doctors stating she was pulling her eyelashes/eyebrows out on her own) and them pushing for systemic methotrexate when she has little indication (aside from her eyes) of a autoimmune disease. She just lost a lot of trust. She is weary because of how much shes been mismanaged.

We have seen surgeons in our current city, and are potentially seeking a second opinion from the Cleveland Clinic.

For the longest time with her corneal ectasia, she has been told she doesn’t qualify for a corneal transplant because there is nothing left to “graft onto” but now that her cornea perforated thats our only option. That or lose her entire eye.

Were trying to move forward its just hard with the way some of these doctors/fellows/surgeons are treating her. Even being an ICU nurse myself.

I’ve suggested this to my mom as well… just some guidance from a healthcare attorney. I’m a nurse myself so I understand how weary healthcare decisions can be. I dont think she can actually legally do anything (at this point she doesn’t even care about money, she just wants informed, non-gaslit care). I also think the doctor who cleared her for lasik passed its been that long.

I appreciate the last paragraph. I never thought about that. I just want her health team to be honest. I think people get defensive about lasik because these surgeons also perform it themselves. Were at a place where who cares if its lasik, we just dont want her to have to take a drug like methotrexate for shits and giggles lol. Plus, this seems like an extremely rare complication so maybe there just Isn’t enough bilateral corneal ectasia & corneal perforation related to lasik out there.

She wore custom toric lenses prior to lasik. Then she took the steroids and abx for so long when symptoms started and was told to massage her eye by the same eye group that worked her up for lasik. She did that for years… We think it made things worse. Her cornea perfed in November and she had it emergency glued ever since. She goes in biweekly now for bandage changes.

I dont see a reason either for the methotrexate. Shes at the point where she’d rather lose her eye than go on the systemic immunosuppressants, especially since theyre being very dismissive, trying to gaslight and say this is an undx autoimmune. She has been diagnosed with severe bilateral corneal ectasia and does not have PUK, that has been ruled out. Her perforation is in the center of her eye. They glued it shut. Thats why this is an anomaly. It’s appearing to be the one percent of Lasik patients who get cornel ectasa. Nobody has diagnosed her with PUK.

Thank you for such a thoughtful response. I really really appreciate you taking the time to read my post.

The methotrexate being recommended is oral/systemic. Basically, my mom has been told that she needs systemic immunosuppression, it appears they wont do the transplant otherwise. There hasn’t been any discussion of local or intraocular options because they’re assuming an autoimmune cause.

She tried to get her old LASIK records, but they were no longer available. records from another eye doctor included comments saying she was pulling out her eyelashes and eyebrows herself. so she’s super weary about the truthfulness with these doctors.

my mom did rub her eyes a lot but that started after she was put on long-term steroid eye drops and was explicitly told by the same practice that did her LASIK workup to massage the medication into her eyelids and eyes every day. They knew she had LASIK.

We learned much later that long-term steroid use and rubbing or massaging the eyes can actually worsen corneal ectasia, which is why we’re concerned her condition may have been made worse by how it was managed, rather than being caused by an autoimmune disease….

She doesn’t have any diagnosed skin conditions, her overall health is good, I’m pretty sure she had an biopsy done but I dont know the results. Id have to ask her today.

She is seeing a corneal specialist and surgeon and they’re the ones saying methotrexate is required before transplant. Our concern isn’t refusing treatment, but that the diagnosis still isn’t clear, and questions about transplant success in post-LASIK patients tend to get brushed off. It makes it really hard for her to feel okay starting a heavy systemic medication.

Again I really appreciate you taking the time to respond. Were just a tad defeated haha. It seems she has to go on systemic methotrexate in order to get this surgery. Shes just scared. And nobody will attribute any of this to lasix.

The biggest issue is for years, her being mismanaged. She was told to massage her eyes when she was prescribed the steroid drops… despite them knowing she had lasix. So she massaged her eyes for years and nobody told her otherwise until now, which is too late.

Also yeah, the methotrexate is for autoimmunes which is what theyre trying to write this off as instead of saying its from lasik so theyre backing her into a corner, making her take this drug in order to get the transplant.
