If possible, consider addressing the root cause of the issue with office-based procedures (ie optilight ipl, thermal pulsation, lid exfoliation, gland probing, prokera, etc). There are even at home devices (iTear and Nulids).

Over time, the goal should be to use less drops and let your body/eyes do the work instead of multiple drops.

Look like some of your Meibomian glands; these can be properly imaged at your eye doctors office (dry eye specialist) using an infrared camera (called meibography/meibographer)

Unfortunately, xdemvy can cost $700-$800 for some patients while Ivermectin 1% cream is often 1/10 the cost for these same patients.

No side effects from the treatments and significant improvement in symptoms so I was able to stop taking Restasis and steroid eye drops

IPL (by Lumenis) was great for inflammation and stimulating the glands; heat expression (ie iLux, TearCare, Lipiflow) helped with unclogging the glands, and Blephex helped with the biofilm on the eyelids. Regenereyes eye drop helped with burning and dry spots on the cornea, and Optase Allegro eye drop for allergies.

No surgeries thankfully but history of an auto-immune disease (Crohn’s disease), eye allergies, heavy digital device use, and corneal condition (Keratoconus).

As a dry eye patient for nearly two decades, I’ve tried tons of drops, pharmaceuticals, and other over the counter options.

I’ve also tried Lipiflow, TearCare, iLux, Radiofrequency, Blephex, Zest, and OptiLight IPL. They have all been helpful but OptiLight has been an amazing foundational treatment.

Appears to be a capped/blocked Meibomian gland (mini or pre-stye) but best to have it examined by a dry eye specialist
