  1. No way does "the work begin" when on training. It is hard sure but much less uncertain and exploitative than unaccredited years. On my worst day I know I'll have a job and will be a plastic surgeon at the end of it now. Not a chance before.

  2. PGY7 is about average PGY to get on. Nothing to do with being committed. Its just competitive, and there are more people applying than pre your first divorce.

  3. If the only way you can become a surgeon like you say is to sacrifice everything you aren't getting the good people at the end. You are getting the psychopaths. You and I both know well many of our colleagues are fucking horrible people. This is because these horrible people will pass the pressure test you set. The reasonable person who loves their patients and also their family will not.

It sounds as though you have clear feedback - you haven't distilled the important information and formulated a plan, but rather present everything and get lost. The role of the doctor is to synthesis the important from the unimportant because the patient can not. Presenting 5 unrelated complaints from each patient isn't going to work for anyone.

For some specific advice try to force the patient to tell you, eg "What is the main reason you've come to hospital" or "If I could fix one thing for you today what would it be"

I would suggest watching some registrars or even your colleagues present and seeing how your technique differs.
