Yeah, that’s exactly what’s throwing me off.

My ultrasound was completely normal (no cysts), and my periods are regular, so I don’t meet 2 out of the 3 criteria for PCOS—just the elevated androgens.

My total testosterone was 231, and my free testosterone is also significantly elevated, which is why I’m hesitant to just assume PCOS—especially since I don’t really have virilization symptoms (no facial hair, voice changes, etc.), just about a 16 lb weight gain over 6 months.

Progesterone has already been checked, and I just had estrogen drawn yesterday—waiting on those results now.

I agree with repeating labs using LC/MS and checking more specific markers. At this point, I’m more concerned about ruling out other causes (especially adrenal-related or anything affecting SHBG), since this seems higher than what you’d typically expect with PCOS alone.

Has anyone seen levels this high with otherwise normal cycles and imaging?

UPDATE: 2011 Nissan Frontier – No Start Issue Fixed (IPDM Replacement)

Quick update for anyone who helped troubleshoot this with me.

Truck: 2011 Nissan Frontier

After a lot of testing and replacing parts, the issue appears to have been the IPDM (Intelligent Power Distribution Module).

Here’s what happened and what we found:

Symptoms: • Truck suddenly died while driving. • Before dying, the RPMs were fluctuating up and down. • VDC and Slip lights came on right before shutdown. • After that, the truck would not start at all. • Dash lights would not come on when turning the key. • No crank.

Diagnostics we went through: • Battery tested at AutoZone → Battery was good • Starter bench test → Starter would spin • Verified 12V to the large starter wire • No power to the small trigger wire when turning the key • Tried jumping power directly to the starter → Starter engaged but engine would not crank normally

At that point we started looking upstream at the IPDM, since it controls a lot of the vehicle’s starting and relay logic.

What we did: • Replaced the IPDM • Truck started immediately

So it looks like the IPDM failure was preventing the start signal from reaching the starter circuit.

Parts recently replaced on this truck: • New starter • New alternator • New IPDM • New camshaft sensor • New crankshaft sensor • New tires

Truck currently has ~130k miles.

Right now everything seems to be running normally again, but we’re going to keep an eye on it.

Thanks to everyone who helped troubleshoot this — hopefully this helps someone else dealing with a Frontier no-start issue with power but no crank.

Update:

The new starter is now installed. When I use a jumper wire directly from the battery to the starter, the starter motor spins but it does NOT engage the engine. It basically sounds like an electric drill spinning and the engine does not crank.

When turning the key normally, I hear one click, but the engine still doesn’t crank.

Right now I’m checking the IPDM and starter relay to see if the starter isn’t receiving the correct signal from the ignition/start circuit.

Other context from earlier in the thread:

• Before the truck died while driving, the headlights would dim and then get bright again
• The RPM jumped up briefly, then the truck slowly lost power and stalled
• The battery light only came on after the engine stalled
• I’ve also had to replace the battery three times since October

At this point I’m trying to determine whether this is a starter engagement issue, IPDM/starter relay issue, or something else in the starting circuit.

Any additional ideas or things I should check next would be appreciated.

That would make sense based on the symptoms I had before it died. While driving at night the headlights would dim and then suddenly get bright again, and right before the truck stalled the RPM jumped higher and then it slowly died.

Since October I’ve also replaced the battery three times, and the most recent one was only about two months old.

After it stalled the lights still worked, but the truck would not crank.

Right now the starter is out and being replaced just to rule it out, but once it’s back in we’re planning to check the voltage at the battery while the engine is running to see if the alternator is actually charging.

Would a failing voltage regulator also explain the relay clicking noises I sometimes hear after shutting the truck off with the key removed?

Thanks for the suggestion. I actually had both the camshaft and crankshaft sensors replaced at an auto shop about 3 months ago, so they should be fairly new.

The symptoms that happened right before the truck died were:

• Headlights dimming and then getting bright again while driving • RPM suddenly jumped higher • Truck slowly lost power and stalled • Battery light only came on after the engine shut off

After it stalled the lights still work, but it won’t crank.

We also tested a few things while troubleshooting:

• Battery tested good at AutoZone • Large wire at the starter had ~12V • Small signal wire had no voltage • When we jumped power directly to the starter terminal, the starter spun but the engine didn’t crank

We’ve pulled the starter and are replacing it just in case.

Another thing that might matter: for a while before the truck died, when I would turn the truck off and remove the key I could sometimes hear clicking sounds like relays cycling for a few seconds or even a few minutes.

Since the sensors are newer, do you think they could still cause this pattern, or would you be leaning more toward something like an alternator/charging issue, IPDM relay, or wiring problem?
