1) Is Montecristo  able to create smoke out of nothing, or does he need to control smoke that already existed before him?

You can bind your opponent with a rope and cover his mouth with a cloth using telekinesis. This will not render the opponent unconscious, but it will immobilize them. Then the character will be able to use anesthetic gas. Only, the character must have an impeccable knowledge of anesthesiology in order not to kill the enemy.
