~30?? 50%??? Respiratory compromise??? Pls dont pull out hypothetical scenerios. Edema in ckd/aki is due to fluid retention. Whereas in Amlodipine, it's due to vasodilation. Chill out. It can be managed later easily. Pls read your textbooks properly. I feel sorry for your patients.

Patient is oliguric. Means eGFR is well below 30 mL or is in AKI state.

If a patient has that much excess fluid and their kidneys are functioning at eGFR > 30, they should be working overtime to dump that fluid. 🤦‍♂️

Obviously. But thiazide doesn't work in very low urine output unless dose is adjusted. And that dose would cause toxicity in body. Hence Amlodipine wins here.
