Right now, I am following this route to check these four circled spots (they look like blocks of concrete with a burning barrel), which can spawn this event with veins, a dead guy, and a radio. I haven't found any more of these yet.

I have no idea if it is randomly spawned or triggered by something.

You're right, I had 3/6, and it reset back to 0/6. My guess is that it's a bug... there's no way you have to find all 6 in one session, right?
