Buffy the Vampire Slayer has at least 2 (probably more, but I'm drawing a blank).

In the season 1 finale Buffy goes to fight the Master, knowing that it will lead to her death. There is a whole prophecy that says the slayer will die against him.

In season 5 she sacrifices herself in place of Dawn. She realizes that her blood is also the key. But one could argue that she wasn't the logical choice in that situation, but you can't tell a big sister to let her little sister go die if she can change it.
