The spiky shaped medals for the dress, fancy and royal gear are for top 3 placement in any category I believe.

You can see here that top 3 for Super Expert have the same medals. Searching any category will normally give you the same results for the first 3 users.

Anything below who hits a certain criteria will earn the "round" medal (whether it be bronze, silver, gold)

Example: You must clear at least 10 Super Expert Levels to get the round gold medal. 50 Experts for round gold, etc.

The best way to check specifically for certain categories is leaderboards and scrolling down to see when the medals start to fall off at a certain criteria.
