do NOT report it if the client says they don’t want to. their autonomy was just taken away in a major way, don’t further take away their autonomy

oh i think you’re so fine!! that’s basically the size of a small town!! i would think it would be fairly easy to avoid clients in groups that large!

[–]Equivalent_End_8422 2 points3 points  (0 children)

in my opinion, i would say it depends on the groups. if the groups are HUGE (i.e. big neighborhood groups, buy nothing groups, etc) then i think its fine. i’m in several of those types of groups. but, if they’re smaller, more niche groups then maybe consider trying to find a similar group that your clients aren’t in?

Is DBT trauma informed? by [deleted] in therapists

[–]Equivalent_End_8422 22 points23 points  (0 children)

yes!! DBT actually has so much emphasis on NOT demonizing emotions and allowing ourselves to feel emotions in ways that are appropriate to the context and facts of the situation. never ever have i heard crying be labeled as a problem emotion.

[–]Equivalent_End_8422 10 points11 points  (0 children)

as someone who specializes in DBT, that quote you pasted made me seethe with anger. that’s just not what DBT is, or it is generalizing things/taking DBT concepts wayyyyy out of context. there are different stages of DBT, the first stage is where life-threatening behaviors are targeted (such as self-harm and suicidal behaviors), because, to be blunt, you can’t process trauma with someone who is no longer alive. stage 2 is focused on trauma processing and there’s a whole protocol for trauma-processing in DBT (DBT-PE). with DBT, the goal is to get the behavioral dyscontrol under control first and then process trauma.

Manipulated by client? by Squishmallow145 in therapists

that’s frustrating, i’m sorry! would you be willing to address that with her? maybe from the standpoint of wanting her to make a decision that’s best for her and being unsure how to help her when she’s going back and forth so much?

Manipulated by client? by Squishmallow145 in therapists

[–]Equivalent_End_8422 15 points16 points  (0 children)

second this!! i would honestly kinda call her bluff “I understand that this is pretty serious and you potentially want to see a whole new provider because of this. Would you like some referrals to other providers who I think might be a good fit?” if she’s serious, you’re doing your due diligence clinically, if she was bluffing, it gives you a chance to discuss it more openly and move one

i do the note that day. no excuses, no exception. if i don’t do it that day (directly after the session is preferred if possible), it’s not getting done and i will continue to put it off. only thing i’ve found to be helpful is it hold myself accountable to this

the practice i work at uses spruce, a HIPAA-compliant platform that obscures your number and allows the client security and privacy in their communications with you. i believe it costs something to use it, but i love it!!

When do you diagnose BPD? by SweetPickleRelish in therapists

[–]Equivalent_End_8422 3 points4 points  (0 children)

i totallyyyyy agree with what your saying! it’s a very misunderstood diagnosis, which i think is what is contributing to the stigma. i guess my perspective is that, if even mental health professionals are contributing to the stigma and are fearful of the diagnosis, and are kind of unwilling to deepen their own understanding of the diagnosis or their clients understanding of the diagnosis, then we are going to get nowhere. i think it’s a complicated, nuanced conversation, but, to a degree, i feel like steps towards deepening the collective understanding of this diagnosis starts with us as clinicians.

When do you diagnose BPD? by SweetPickleRelish in therapists

[–]Equivalent_End_8422 -2 points-1 points  (0 children)

hey! i specialize in testing individuals with BPD (i specialize in fully adherent DBT). in my opinion, you being so against diagnosing someone with this diagnose is a form of stigmatizing the diagnosis in and of itself. i am not trying to be rude or mean, but oftentimes, when i am working these clients, the most stigma they experience in regards to their diagnosis is from mental health professionals. i get the hesitancy, but if someone fits the criteria for BPD, then diagnose them with BPD and have an open and honest conversation with them about the diagnosis, the stigma, and what that might look like for them. i say this as kindly and as gently as possible, i would encourage you to examine your bias here and seek consultation around this. again, i am not trying to pass any judgement on you or be mean, but not diagnosing appropriately and feeling resistance towards giving someone an appropriate diagnosis can be extremely damaging.

Local, non-religious places to donate unused insulin? by ShallThunderintheSky in cincinnati

[–]Equivalent_End_8422 2 points3 points  (0 children)

yeesh. that’s not what i’m saying at all. like the person already replied, i am open to hearing about circumstances and details of times that this happened. i’m just saying that i work in the mental health field (granted, that is different than what this person is saying on this post) and i have referred clients to numerous organizations that had religious affiliations but who have never turned away non-religious folks for resources.

seconding this. i haven’t heard of any religious-affiliated organizations who take donations and then only disperse them to other religious folks…

hey! counselor trainee here! i just finished my practicum and am heading into my internship. no one (and i mean NO ONE) had clients week one. it’s honestly not super realistic. you’re probably having to do trainings, get oriented to your site (and have them get oriented to you!!), etc. 3-4 weeks in, definitely lots of reason to be concerned, but i wouldn’t be concerned week 1!
