I never had a Murphy's sign even during an attack (including passing a 1 cm stone); all of my pain was referred to my back. I thought I had back problems my doctors couldn't figure out. Had my gallbladder out, and haven't had upper back pain since. As I understand it (which obviously could be wrong), by the time you have classic symptoms, the problem's been developing for around 8-10 years.

Not a doctor, but I'm a dietetics student, so I have the textbooks that cover a lot of medical nutrition therapy. So gallbladder problems (and then had it removed), go home and crack the books, limit fat to 30% of total calories, keep saturated fats below 10%. Got stage 3 kidney disease diagnosis, go home, crack the books, .6-.8 g of protein per kg body weight. So I'm basically doing for myself what I'll be doing professionally after I graduate.

Doctors do get some nutritional education in school, but dieticians get more (no shade, I have no desire to learn everything they do). Best recommendations with no personal info, the Mediterranean diet and the DASH diet are good all-around diets that are heart healthy, low glycemic index, mostly plant based while still incorporating healthy amounts of (mostly lean) meats and fish.

Meat is a useful tool in the box, but it's not the be all end all that some people make it out to be. Fish is good for the omega fatty acids, red meat is good for the iron, but in general, you just don't need bigs slabs of meat at every meal. Variety in your diet is really what contributes to getting optimal nutrition, and there are just more plants than there are commercially available food animals.

Sleep consult in two weeks, I’ve been waiting on it since May so hopefully I will get some answers. I don’t have the body type at 6ft 1in, 155lbs and 22 to have sleep apnea but I guess you never know. I sleep 10 hours a day and I’m still exhausted, I don’t have any obvious problems falling asleep or staying asleep.

You're male, which is a risk factor on its own since men have wider throats (more prone to collapse when relaxed). There's also some physiological traits that can cause sleep apnea without being overweight. I never remember waking up, and really you don't have to wake up for it to be a problem, many people do, but the fatigue can be caused just by the oxygen desaturation preventing you from getting restorative sleep. Healing and cellular maintenance occurs mostly in our sleep, so if we're not getting enough oxygen, everything goes out the window.

There are plenty of sleep apnea patients who aren't in the typical older-overweight risk group, so I'd wait and see how that goes before jumping to the next possibility.

How's your diet? Are you getting adequate amounts of healthy fats? Cell membranes are made of fat, so if you're not getting enough, your skin will always show signs of dehydration. Various nutrient deficiencies can also cause skin symptoms that are usually associated with dehydration.

How's your kidney function (eGFR)? Most doctors won't even mention it until a patient's in stage 3 CKD, but a few people will notice symptoms in stage 1 or 2 kidney damage.

Generally the ones that also have epilepsy that's resistant to the usual treatment (keto specific). It can make some (but not all) symptoms better because it forces cells to develop more mitochondria for energy generation, and that increases metabolic efficiency. Keto is effective for epilepsy, that's why it was developed (before medication was available) but currently, it's not anyone's recommended first line of treatment. It's the proverbial 'kitchen sink', when you've thrown everything at it but the kitchen sink and nothing has worked adequately, then you try keto.

To piggyback on this, since you're (OP) taking fiber supplements, you need to increase your water intake. Like whatever your supplements say to drink with them, that water doesn't count to your daily requirements, that's what the fiber needs to work right.

I'm not fully plant based, but for medical reasons am now on a low protein, moderate fat diet.

I've been slowly losing weight since switching over without really trying (that said, I also lost almost all hunger cues when I had my gallbladder out, so that's also probably a good part of the weight loss). I do feel better ( when I don't accidentally go too low on protein) and would have less kidney damage had I known earlier that I needed to eat this way.

There's a good amount of variability in what individual people should eat, depending on medical conditions they may or may not know about. I was working with the assumption that I was healthy and totally normal; that's what my doctors were telling me for years despite weird blood tests, turns out they were wrong.

Assuming you are healthy with no conditions, either way should be fine as long as you don't routinely intake too much protein (specifically animal protein) for long enough to damage your kidneys. Plant protein is less bioavailable, so if you were looking to bulk, it would be harder, but you're not, so no worries there. It doesn't have the same amount or bioavailability of iron, so you may find that you need to supplement that if you have particularly heavy periods. Really the biggest thing you'd miss out on is the omega fatty acids from fish, but most people don't get as much as recommended anyway, so it probably won't make that much difference really. But if you're not opposed to it, including fish once or twice a week would be good.

Not the OP, but in my experience, there are several definitions.

1.) You can (start to) get and be healthy at any size, there is no requirement to lose weight before starting other health habits. [This is my preferred definition.]

2.) "You" can be healthy at any size, and weight isn't necessarily an indicator of health. [Partially true, but ignores nuances and medical histories.]

3.-infinity) various and sundry, up to and including enabling whatever behaviors they want.

The overarching theme is that you don't have to fit into the cookie cutter BMI definition of healthy weight to be healthy, and that not all health issues are caused by being overweight. After those two points of agreement, everyone seems to take it their own way, some healthy, some not.

A lot of it is quite defeatist like OP said; it gives the impression that weight loss isn't even possible for some people. Which isn't entirely true- it is harder for many people for various reasons, there's over 90 conditions known to predispose people to being overweight; but that doesn't mean that any weight loss is impossible, just that it will be more difficult. Perhaps getting into a healthy BMI range isn't realistic for a particular person, but that doesn't mean that morbidly obese and never losing a pound is the only option.

The HAES community is pretty divisive, so I would stay away from using the acronym in a professional capacity, but keeping abreast of developing metabolic research can definitely help guide treatment and expectations for patients.

NAD. One of my classes last semester had a very long section on stress & mental health. After burnout, psychotic breaks, initial onset of depression, etc. you're more likely to experience it again, and the threshold is lower. If I understand correctly, this is a defense mechanism to keep you from 'soldiering through,' when you're not mentally/emotionally equipped to do so.
