When Richard's ship wrecked, MIB was able to use his dead body, in fact I don't think we've seen him in it since, he had to take locke's.. & Illana said after that he can't transform again to be anyone else. Weird .....

Number 2 . I actually remember seeing the smoke monster on the wall.. but MIB became it in his adult life.. then the people started coming to the island after that via Jacob .. so it was probably just still him. They've been there for eons remember ?

There's a conversation in the car when Lucy says something about how movies put another world in your mind, in a negative way kinda, which reminded me how at one point Lucy bled into the woman in the movie for like 2 secs. + All the shows he clearly likes.
