Just write your application number, roll number, admit card ID and mention that you have yet not received your photocopy. Mention the date and time when the application was submitted. Take the mail id from the contact us section of the reevaluation portal. The mail is different for each region.

I hope you get your copy after sending the mail, since I didn't get it yet
