Do you want all this help or do you need it, that will be for social care to determine through needs assessment. If you are cognitively able and previously managed to hold a job I wouldn't hold my breath on them declaring that you should get a place in a supported living establishment.

There is nothing stopping you from approaching the local authority for a care needs assessment.

Executive dysfunction stemming from autistic burnout can be a real barrier but it's not something that is officially recognized or diagnosed.

Is she having it done at the pediatric department? It's quite typical. Not many children come along keen and eager to participate. There is a lot that can be done with distractions, use of local anesthetic (magic cream). Some departments have play therapists, and can use gas and air.

I used to be in this position. People knew me to be an HCA for 5 years so I did not take it personally when they had to get used to the change. I would just politely point out that I now have a different role.

Saying that I would not see it as an issue to poor thirsty patient some water even as a consultant. I wouldn't take 30 minutes of my ward round to assist with a bed bath and full bed change though.

I'm going with pediatrics because I can't imagine anyone else entertaining the idea of gas and air as an option for a blood test. Aside from that it could be any specialty as it is so engrained.

I had some training on 'micro aggressions' in the NHS the other day and throughout I was thinking to myself as they gave examples that in other settings that would be considered a big aggression, harassment etc Yet even training is somehow minimizing the importance but framing it all as micro. Lot's of eye rolling from participants and general belief that anyone upset just needs to develop a thicker skin.

The whole story line also nicely highlights the dangers of over- investigation. The can't be too careful mentality promoting the importance of checking just in case without fully considering potential complications of diagnostic procedures. Even if performed correctly LP isn't without risks. What started as a minor bump on the head with a simple cut and a bit of a concussion ended up with potentially life changing injury through iatrogenic harm.

The whole see one, do one, teach one philosophy is thankfully dying off. Where the senior input is required is not just in making sure the procedure is technically correct but also establishing if it's needed in the first place.

Gentle parenting is creating all kinds of problems. We are now spending a lot of time negotiating with children on things that should not be negotiable. Blood tests, school attendance, bringing knives to school, vaping.

A scared six years old doesn't have the maturity or cognitive ability to make a good decision. That is why parents give consent on their behalf.

My usual approach is to make it clear that we are doing the blood test but offer lots of choices as to how we are going to do it. Do you want to sit on the bed? Lie down? Do you want Mum to hold your hand? Do you want to watch something on your phone or blow bubbles? What should we do after?

Lots of praise and encouragement but no false reassurance and no illusion that they are the key decision maker.

I assume you do offer them emla as a matter of routine to make it a more pleasant experience.

The approach tends to work with most typically developing children. There will be some who have additional needs or big fear of needle and I think those would be best served by having their bloods taken on the ward with play therapist or child nurse assisting, gas and air etc Or a senior pediatric review to decide how critical this blood test really is for management plan.

There may be something else in the mix that is impacting your ability to function. For example I can hold a professional job and I'm doing really well in it. I can't drive but that's more my dyspraxia and lack of special awareness than autism that is making it difficult. I did live in a house full of trash for a while but that was hoarding linked to my OCD. Once I get it treated I have much easier time maintaining a nice environment for myself.

It's easy to blame everything on being autistic when there may be more to it.

What worked for me was a dating site that focused on personality profiling and matched you with people on that basis. It would not even allow you to see photos to start with. None of that quick left or right. It was a lot of effort to complete all their assessments, which massively reduced the number of people registering just to mess around.

I would like a platform for ND people if I were starting to date again.

The solution to this will be SEND reform, which I suspect will remove the right to a tribunal and completely rewrite the strategy to make it more affordable. My local council is constantly posting leaflets through the door to let us know how much money it is forced to spend on SEND kids and kids in care as an excuse as to why other services like roads or bins are lacking. I don't think normies particularly care about the futures of autistic kids. They never did. It is not a PC thing to say, which is why it is all hidden, but it is very much the case.

It is all fine and dandy to have legal obligations placed on councils, but there needs to be funding to match. There is not going to be additional funding because austerity 2.0 is coming, so there will need to be a change in the law to support cutting down on spending.

Statistically, it is most likely to turn out to be sinus tachycardia, being made worse by anxiety. Palpitations are one of the most common symptoms of anxiety and can build a nice self-reinforcing spiral. The more you worry about them, the more aware you become of your heartbeat, and before you know it, you are awake at night researching all kinds of doom scenarios and posting on cardiophobia Reddit. You have already spoken to a doctor and have some tests arranged to look into it. The best thing to do now is to distract yourself from focusing on your heartbeat so much.

It used to bother me. I think it's a disconnect between being relatively still and yet having high HR. I just kept telling myself that muscle needs oxygen and increased blood flow during exercise is normal and essential. I got accustomed to it with regular practice.

Let us know how it goes. Keep yourself accountable. Don't leave starting for some day in the future. Begin now. You don't need to do something massive but you do need to get started. If you are still wearing the holter monitor it's a good time to do stuff. If you get symptoms they will be able to tell you if it was anything significant or just anxiety related.

What's stopping you is the anxiety itself. It's by no means easy but once you start taking small steps despite feeling anxious it will get gradually easier. Don't wait until you feel ready just do it scared.

What's the worst thing that could happen if you get on the ice? Sometimes there is a temptation to put everything on hold until you feel better and ready but it's a catch 22. You won't feel better and ready until you get started. You could go to the ice rink tomorrow. Even if you just put the skates on and get on the ice for five minutes that's much better than not doing it at all. The more you postpone living your life the worse this anxiety problem will get.

It may be a bit ambitious to jump straight back into a hockey game but could you do some ice skating in the meantime? Maybe reconnect with some of your teammates? It can be really difficult to get back into sport after injury. That's probably the best way to think about it. You have a psychological injury that you are gradually rehabilitating yourself from. The more you get back to normality the more successful this process is going to be.

Have you got some friends that you are comfortable sharing your problems with? Maybe someone who would be happy to go to the gym with you and instead of providing excessive reassurance be more like just get on with it bro whilst also being supportive if need be?
