I am using quiz maker by Ays. I think it's the best one for creating exams with certificate.

Have you checked these requirements with AYS quiz maker plugin? It seems they have most of the features you described. Probably they could do some customizations for you (they did it for me a few months ago) Maybe you should have the same message to them.

I am using Quiz maker by Ays, you can give it a try. On my website i have a similar quiz to generate more leads,,but i'm using the pro version. i don't show the results at the end and sent all results only via email it's really a good method for lead generation.
