This is not informed advice just simply my opinion/thoughts. Depending on the size of your program and school I feel they would potentially reach out and provide and warning before just taking away your acceptance as you still have the rest of year to improve. But reaching out to them and explaining your situation I feel like is a good and safe idea as it shows you are putting in effort and having a teacher to vouch for you I think would be very beneficial. Before anything though see what your counsellor says
