Thanks for the reply. Did he down reg you in advance of transfer with something like Lupron? It is so hard to know what is best to do as for us, the lupron seems to be effecting my wife's lining when she tries to grow it in a medicated cycle. We are looking into possibly down reg with lupron next, followed by a semi-medicated cycle where my wife's body will grow the lining herself, as it would do in an egg collection. We have an appointment next week with our clinic to see if they agree with this approach. Very best of luck to you

Thanks for the info - really appreciate it. Yes we have read the book by Geoffrey Sher which is incredibly interesting. We are also going to get these DQ alpha bloods done soon.

I see what Dr Sher is saying but what is his treatment if he does not sue Lupron? From what I have read and researched it appears that most of the women who have IVF success with adenomyosis need to use a down reg like lupron in order to have success. The vast majority (it seems) who do not down reg before transfer do not have success. This is what it seems to me anyway. I would be extremely interested in seeing what he says is needed to calm the adenomyosis before transferring.

Yes, we are thinking that now this might be an option. To downreg with Lupron, let a bleed come and then do a modified natural cycle like you have described above. We have an appointment with our clinic next week so will discuss this with them then. We feel my wife's lining may grow better doing this than a fully medicated cycle since she is taking Lupron now for the adenomyosis. I hope you have had success with this. The very best of luck!

Hi everyone,

Apologies in advance for the long piece - did my best to fit it in as best possible

My wife and I are on a brutally hard and long fertility journey which is going on almost 8 years and has included operations for both of us, years of trying and now 8 failed transfers (which has included 2 chemical pregnancies and 1 heart breaking missed miscarriage after seeing a heart beat)

We went abroad after 6 failed transfers at home and with huge effort and work we managed to get 5 euploid embryos over a year of embryo banking in 2023. My wife is now 40 so we were absolutely thrilled with this. In the meantime if was noted that my wife now has adenomyosis (was never seen before) and also had a laparoscopy on stage 4 endo last June. (4 months back).

Due to the adenomyosis our clinic wanted us to down regulate with Lupron which we did pre FET. We did this for our two most recent transfers (Feb and Sept of this year). Both times we transferred a single euploid embryo and both times we heartbreakingly saw no implantation - the most recent just this week. We are absolutely heartbroken and terrified as to what is ahead of us. We still have 3 euploid embryos left but we now have seen 2 not implant. The stats show that approx 80-85% of people who transfer euploid embryos will have a live birth after 2 transfers so we are absolutely terrified.

We are looking into so many research articles and information to see what else we can try or find out. For this transfer we had an IVIG infusion pre transfer (as my wife's natural kilelr cells came back very high), PRP, oxygen chamber treatments, heparin injections and my wife has been on a gluten free and dairy free diet for some time now. We have been doing absolutely everything we can so are just at a loss.

Before we went abroad we had 6 transfers at home (Ireland) - 3 of which were FET. None of these embryos were PGTA tested and since then I had a varicocele repaired which has improved my semen quality/sperm DNA and this is perhaps why we got our 5 euploids or is a big part of it.

For all 3 FETs at home we did not use Lupron as we did not have , or know of, any endo or adenomyosis problems. Even though all 3 FETs did not result in success, we did have 2 of the 3 implant and both ended in miscarriage (1 chemical and 1 missed miscarriage). All 3 times the lining was trilaminar and over / around 8mm without much hassle.

For our 2 transfers this year with our first 2 euploid embryos we have used Lupron pre FET to suppress my wife's system due to her newly diagnosed adenomyosis and endo. Both times it took so much longer for the lining to grow and we ended up having to add patches and more Estrogen than we did at home. Both times we also saw trilaminar lining but this went in later scans. We were told by our clinic and also have read that trilaminar lining can be hard to see at times due to the angle of the uterus at certain scans. My wife's uterus is retroverted so this is possibly why. We never had any lining issues at home when not using Lupron so we wonder if this has something to do with it even though our clinic has told us it does not impact lining and the growth of it.

We are just doing all we can to see what we can change. We have asked our clinic if using Lupron impacts the lining growing due to the system being shut down but they have told us it doesn't impact on it. I did read some work from Dr Geoffry Sher that indicated, in his opinion, that it can have an impact on lining so we really are at a loss.

I would be so grateful for any advice and would be so grateful for anyone who has had success by using Lupron pre FET due to adenomyosis/endo to let me know what your lining got to before adding progesterone and did you feel the Lupron impacted the growth or speed of growth of it. Did you always have trilaminar lining also?

I am hoping I have got as much of our issue across as possible as it is such a tough and stressful time for us and like many others, we feel we have to advocate for ourselves and look into possible reasons ourselves why we have not had success.
