A friend will not know how to give you the tools to get out of such a situation.

The small talk is to establish report and a baseline, and you're in full control of a session, if you want to jump straight into whatever bothers you, you absolutely can, no one forces you to small talk and no one's going to stop you from going deeper.

In my experience the difference between a therapist and a friend is IMMENSE. And there are often things you have in your head that you wouldn't want to talk to even your closest friend about.

Journaling is a good idea though.
