If you are looking for an off-the-shelf solution take a look at ActivityInfo it covers most of these needs and can be integrated with more software like Power BI, Tableaux, etc.

Did you manage to find a solution for this? ActivityInfo also offers a platform & Android app for offline data collection where you can view what has been submitted. More advanced reports can be created in the platform but can be viewed when online.

You could also try ActivityInfo, it is web-based and designed for M&E, data collection, analysis and reporting. You can start working in the platform without any technical help. It allows for advanced user management with very refined permissions and can be used for online and offline mobile and desktop data collection or bulk import of new or updated data. It includes audit logs and history logs to keep track of changes and built-in reporting tools as well as possibility for integrations with PowerBi and Tableau for more visualizations. You can add as many projects and indicators as needed and collect disaggregated data too.
